儿茶酚氧位甲基转移酶基因Val/Met及Ala22/72Ser多态性对无抽搐电休克治疗精神分裂症患者认知功能的影响

【摘要】 目的 探讨儿茶酚氧位甲基转移酶基因Val/Met、Ala22/72Ser多态性对无抽搐电休克治疗精神分裂症患者认知功能的影响。方法 检测50例精神分裂症患者的基线儿茶酚氧位甲基转移酶基因Val/Met、Ala2272Ser多态性,并予以无抽搐电休克治疗,于治疗前后进行认知功能测评及神经心理测验,比较治疗前及治疗后不同基因型患者认知功能及神经心理测验结果的差异。结果 治疗后入组患者P300潜伏期N1较治疗前显著缩短(P<0.05),波幅P3较治疗前显著增高(P<0.05),波幅P2与治疗前比较差异无统计学意义(P>0.05);颜色连线、Stroop测验的单词、颜色、色词及威斯康星卡片分类测验的正确应答数、持续错误数、概念化应答数均较治疗前显著改善(P<0.05或0.01)。治疗前及治疗后,Val/Met、Ala22/72Ser不同基因型患者P300及神经心理测试结果比较差异均无统计学意义(P>0.05)。结论 无抽搐电休克治疗对精神分裂症患者的认知功能有改善作用,但在儿茶酚氧位甲基转移酶Val/Met、la22/72Ser不同基因型患者认知功能改善方面无明显差异。
【Abstract】 Objective To explore the impact of catechol oxygen methyltransferase gene Val/Met and Ala22/72 Ser polymorphisms on cognitive function in patients with schizophrenia treated by MECT.Methods Baseline MECT gene Val/Met and Ala22/72 Ser polymorphisms were detected in 50 patients with schizophrenia and treated with MECT.Cognitive function evaluation and neuropsychological test were performed before and after treatment,and the differences in cognitive function and neuropsychological test results of patients with different genotypes before and after treatment were compared.Results After treatment,patients with P300 latency N1 were significantly shorter than before treatment(P <0.05),and amplitude P3 was significantly higher than before treatment(P<0.05).There was no significant difference in amplitude P2 compared with before treatment(P>0.05).The color connection,the words of the Stroop test,the color,the color words,the number of correct responses,the number of per sistent errors,and the number of conceptual responses of WCST were significantly improved compared with before treatment(P <0.05 or0.01).There were no significant differences in P300 and neuropsychological test results between patients with different genotypes of Val/Met andAla22/72 Ser before and after treatment(P>0.05).Conclusions MECT treatment can improve the cognitive function of patients with schizophrenia,but there is no significant difference in the improvement of cognitive function in patients with different genotypes of COMT Val/Met and la22/72 Ser.
