微创锁定加压钢板内固定治疗肱骨近端骨折

【摘要】 目的探讨微创锁定加压钢板(LCP)内固定治疗肱骨近端骨折的疗效。方法采用微创LCP内固定治疗112例肱骨近端骨折患者。观察患者关节活动度恢复情况,并采用Neer肩关节功能评分标准评价临床疗效。结果 112例患者均获得随访,时间6~12个月。未见明显内固定断裂和螺钉穿出、移位等并发症。术后6个月采用Neer肩关节功能评分标准评价疗效:优60例,良45例,可6例,差1例,优良率为93.75%(105/112)。末次随访时,患侧肩关节活动度:外展50°~70°(62.44°±4.81°),外旋30°~45°(37.50°±7.72°),前屈90°~115°(102.35°±5.68°),后伸10°~20°(14.71°±2.25°)。结论微创LCP内固定治疗肱骨近端骨折疗效良好。

【Abstract】 Objective To investigate the effects of minimally invasive locking compression plate( LCP) internal fixation in the treatment of proximal humeral fractures.Methods The 112 patients with proximal humeral fractures were treated with minimally invasive LCP internal fixation.The patient’ s joint range of motion was observed.The clinical effect was evaluated with Neer shoulder function score.Results All cases were followed up for 6 ~ 12 months.There were no complications such as breakage of internal fixation,screw penetration and displacement.The curative effect was evaluated with the Neer shoulder function score at 6 months after surgery,and the results showed that the curative effect was excellent in 60 cases,good in 45 cases,fair 6 cases,and poor 1,with an excellent and good rate of 93.75%( 105/112).At the last follow-up,the shoulder range of motion on the affected side was as follows: abduction with50° ~ 70°( 62.44° ± 4.81°),extorsion 30° ~ 45°( 37.50° ± 7.72°),anteflexion 90° ~ 115°( 102.35° ± 5.68°)and backward extension 10° ~ 20°( 14.71° ± 2.25°) . Conclusions Minimally invasive LCP internal fixation is effective in the treatment of proximal humeral fractures.
