上皮性卵巢癌肿瘤大小的临床病理特征及预后分析

【摘要】 目的:探讨肿瘤大小与上皮性卵巢癌的临床病理特征及预后差异。方法:收集我院收治120例上皮性卵巢癌患者,其中早期患者42例,晚期患者78例。采用χ~2检验分析肿瘤大小与各临床病理因素的关系,运用Kaplan-Meier法及Log-rank检验进行生存分析。采用Cox比例风险模型进行单因素及多因素分析。结果:早期卵巢癌患者42例,肿瘤平均直径13.5cm,晚期卵巢癌患者78例,肿瘤平均直径7.8cm,差异具有统计学意义。肿瘤≥10cm组与肿瘤<10cm组在患者年龄、组织学类型和CA125水平无明显相关性(P>0.05),而与FIGO分期以及病理分级有明显相关性(P<0.001);肿瘤≥10cm卵巢癌组3年生存率40.4%,显著高于肿瘤<10cm卵巢组3年生存率18.1%,(P<0.05)。单因素及多因素分析显示肿瘤大小是上皮性卵巢癌预后的独立影响因素。结论:肿瘤大小可作为卵巢TNM分期的重要补充,为评估卵巢癌预后提供重要参考价值。

【Abstract】 Objective: To investigate the effect of tumor size on the clinicopathological features and prognosis of epithelial ovarian cancer.Methods: A total of 120 patients with epithelial ovarian cancer, including 42 in the early stage and 78 in the advanced stage, were selected from the first affiliated hospital of anhui medical university.The relationship between tumor size and clinicopathological factors was analyzed by chi-square test, and survival was analyzed by kaplan-meier test and log-rank test. Cox proportional risk model was used for single factor and multiple factor analysis. Results: The average tumor diameter of 42 patients with early ovarian cancer was 13.5 cm, and that of 78 patients with advanced ovarian cancer was 7.8 cm. The tumor diameter of the patients with early ovarian cancer was significantly larger than that of the patients with advanced ovarian cancer, and the difference was statistically significant(P<0.05).The tumor diameter was 10 cm, ≥10 cm group and <10 cm group, there was no significant correlation between age, histological type and CA125 level(P>0.05). ≥10 cm group and <10 cm group, there was a significant correlation with FIGO staging and pathological grading(P<0.001). The 3-year survival rate of the tumor ≥10 cm group was 40.4%, which was significantly higher than that of the tumor <10 cm group(18.1%), and the difference between the two groups was statistically significant(P<0.05).Univariate and multivariate analyses showed that tumor size was an independent factor affecting the prognosis of epithelial ovarian cancer.Conclusion: Tumor size can be used as an important supplement to ovarian TNM stage, and can be used as a reference to evaluate the prognosis of ovarian cancer.
