外周血中前列腺特异性抗原和前列腺干细胞抗原单独或联合检测对前列腺癌的诊断价值

【摘要】 目的:探究外周血中的前列腺特异性抗原(PSA)以及前列腺干细胞抗原(PSCA)单独或进行联合检测在前列腺癌临床诊断中的应用价值。方法:选取本院2016-08~2018-08本院确诊前列腺增生患者50例建立BPH组、前列腺癌患者50例建立PCA组以及同期进行健康体检者50例建立对照组。通过酶联免疫吸附试验检测三组研究对象血清PSA以及PSCA含量,通过受试者工作特征曲线(ROC)评价PSA以及PSCA单独或进行联合检测对于前列腺癌诊断的价值。结果:相比于对照组,BPH组以及PCA组的血清PSA以及PSCA含量均有明显的升高(P<0.01);PCA组相比于BPH组的PSA以及PSCA含量均有明显的升高(P<0.01)。对照组与BPH组中,血清PSA与PSCA的联合检测ROC曲线下面积相比于两指标的单独检测显著更高;对照组与PCA组、BPH组与PCA组中,血清PSA与PSCA的联合检测ROC曲线下面积相比于单独PSA检测均显著更高,但联合检测与单独PSCA检测的对比未见显著差异;三组检测均显示联合检测的诊断价值更高。结论:在健康人与前列腺疾病的患者外周血PSA以及PSCA的表达上存在有显著差异,提示外周血PSA以及PSCA的检测具有诊断BPH与PCA的价值。且ROC曲线分析提示,对外周血PSA与PSCA的单独对BPH与PCA上具有一定的诊断价值,联合检测的效果更佳。

【Abstract】 Objective: To investigate the diagnostic value of peripheral blood prostate-specific antigen(PSA) and prostate stem cell antigen(PSCA) in prostate cancer.Methods: Patients from the first affiliated hospital of jiamusi university from August 2016 to August 2018 were selected as the study subjects. 50 patients with prostatic hyperplasia were in the BPH group, 50 patients with prostate cancer were in the PCA group, and 50 patients with concurrent physical examination were in the control group.The contents of PSA and PSCA in serum of the three groups were determined by enzyme-linked immunosorbent assay.A receiver operating characteristic(ROC) curve was drawn to evaluate the diagnostic value of PSA and PSCA alone or in combination for prostate cancer.Results: The contents of serum PSA and PSCA in PCA group and BPH group were significantly higher than those in healthy control group, and the contents in PCA group were the highest, and the differences were statistically significant(P<0.01).By comparing the area under the ROC curve(AUC) shows that in the control group with BPH group, the serum PSA and PSCA indicators of joint detection diagnosis is better than two separate tests(P<0.05), but the PCA in the group, the serum PSA and PSCA joint detection diagnosis effect is higher than the PSA test alone(P<0.05), compared with separate PSCA detection effect difference has no statistical significance(P>0.05).All three groups showed higher diagnostic value of combined test(P<0.05).Conclusion: There are significant differences in the expression of PSA and PSCA in peripheral blood between healthy people and patients with prostate disease, indicating that peripheral blood PSA and PSCA can be used as markers for the diagnosis of prostate disease, and the combined use of the two has a better diagnostic effect.
