【摘要】 目的系统评价经皮内镜下胃造瘘术对头颈部肿瘤患者的影响。方法计算机检索Cochrane Library、PubMed、Embase、中国生物医学文献数据库、维普信息资源系统、中国知网和万方数据库,收集从建库起至2019年10月国内外发表的关于经皮内镜下胃造瘘对头颈部肿瘤患者影响且符合纳入、排除标准的随机对照临床试验。应用RevMan 5.3软件进行数据分析,采用I~2评估各研究结果间的异质性。结果共纳入符合标准的8篇随机对照研究,总样本643例,结果显示,使用经皮内镜下胃造瘘进行营养支持对患者的体重减少程度、白蛋白下降程度、感染、管道移位、总并发症发生等均有改善(均P<0.05),而对患者的食管狭窄无明显的作用(P>0.05)。结论与其他营养支持方式相比,经皮内镜下胃造瘘能够改善头颈部肿瘤患者的体重和白蛋白减少,减少并发症和不良反应的发生,有利于提高头颈部肿瘤患者后期生存的舒适度。更多还原

【Abstract】 Objective To systematically evaluate the effect of percutaneous endoscopic gastrostomy on head and neck tumors.Methods Cochrane Library,PubMed,Embase,CBM,VIP,CNKI and Wanfang were retrieved from their establishment to October 2019 for the randomized controlled trials of the effect of percutaneous endoscopic gastrostomy on head and neck tumors.Data were analyzed using RevMan 5.3 software,and heterogeneity was assessed by I~2 test.Results A total of 8 randomized controlled trials meeting the inclusion and exclusion criteria(643 patients) were included.Nutritional support by percutaneous endoscopic gastrostomy markedly decreased body weight loss,albumin loss and incidence of infection,tube displacement and total complications(P<0.05),but had no significant effect on esophageal stenosis(P>0.05).ConclusionCompared with other nutritional support methods,percutaneous endoscopic gastrostomy can significantly reduce the weight loss,albumin loss and occurrence of complications and adverse reactions in patients with head and neck tumors.Therefore,it is helpful to improve the survival comfort of patients with head and neck tumors.更多还原
