【摘要】 目的分析外侧裂-岛叶入路和颞叶皮质入路手术方法在高血压性基底节区脑出血治疗中的临床效果。方法将52例高血压性基底节区脑出血患者根据手术入路不同分为2组:经外侧裂-岛叶入路手术组(A组)和经颞叶皮质入路手术组(B组),其中A组30例,B组22例;回顾性分析患者的临床资料,比较2组手术疗效相关指标:血肿量、血肿清除率、手术时间、骨瓣保留率及住院时间;比较2组患者随访6～24个月期间改良Rankin量表(MRS)的评分。结果 2组的血肿量和血肿清除率比较差异无统计学意义(均P>0.05);A组手术时间较B组更长,但A组住院时间较B组更短(均P<0.05);A组骨瓣保留率及MRS评分均优于B组(均P<0.05)。结论经外侧裂-岛叶入路手术治疗高血压性基底节区脑出血的临床疗效优于经颞叶皮质入路手术。更多还原

【Abstract】 Objective To compare the efficacies transsylvian-insular approach and transcortical approach in hypertensive basal ganglia hemorrhage.Methods Clinical data of 52 patients who underwent microsurgery via either transsylvian-insular approach(group A,n=30) or transcortical approach(group B,n=22) for hypertensive basal ganglia hemorrhage were analyzed retrospectively.The mean hematoma volume,hematoma clearance rate,operation time,bone flap retention rate and hospital stay were compared between the two groups.In addition,the modified Rankin Scale(MRS) score was recorded during 6-24 months of follow-up.Results There were no significant differences in hematoma volume and clearance rate between the two groups(P>0.05).Compared with group B,operation time was prolonged,hospital stay was shortened,and bone flap retention rate and MRS score were improved in group A(P<0.05).Conclusion Transsylvian-insular approach is superior to transcortical approach for treating hypertensive basal ganglia hemorrhage.更多还原
