血清PSA联合骨显像在前列腺癌骨转移中的应用
Application of Serum PSA Combined With Bone Imaging in Bone Metastasis of Prostate Cancer
【摘要】 目的探讨血清t-PSA、f-PSA、f/t联合SPECT全身骨显像对老年前列腺癌骨转移的诊断价值。方法收集本院2017年1月—2019年6月收治的70例老年前列腺癌患者,首先行SPECT全身骨显像检查,之后检测血清t-PSA、f-PSA含量,计算f/t比值。结果 70例老年前列腺癌患者中,非骨转移者21例(30.00%),骨转移者49例(70.00%),其中单一病灶者5例(10.20%),转移灶两处者16例(32.65%),三处及三处以上转移者28例(57.14%)。骨转移组的t-PSA、f-PSA高于非骨转移组,f/t比值低于非骨转移组,差异均具有统计学意义(P<0.05);> 2个骨转移灶数目患者与≤2个骨转移灶患者与相比,t-PSA、f-PSA水平显著升高,f/t比值显著降低,差异具有统计学意义(P <0.05);随着f/t比值升高,发生骨转移患者比例减少。结论老年前列腺癌骨转移以多发病灶为主,转移部位以椎体和肋骨为主。t-PSA、f-PSA、f/t比值联合SPECT全身骨显像,在一定程度上能够提高老年前列腺癌骨转移诊断率。
【Abstract】 Objective To evaluate the diagnostic value of serum t-PSA, f-PSA and f/t SPECT with whole body bone imaging for bone metastasis of senile prostate cancer. Methods Seventy elderly patients with prostate cancer admitted to our hospital from January 2017 to June 2019 were collected, and SPECT whole-body bone imaging examination was performed first, then serum t-psa and f-psa contents were detected, and f/t ratio was calculated. Results Among the 70 elderly patients with prostate cancer, 21 patients(30.00%) had non-bone metastases, 49 patients(70.00%) had bone metastases, including 5 patients(10.20%) with single lesion, 16 patients(32.65%) with two metastases, and 28 patients(57.14%) with three or more metastases. The t-psa and f-psa in the bone metastasis group were higher than those in the non-bone metastasis group, and the f/t ratio was lower than that in the non-bone metastasis group, with statistically significant differences(P < 0.05). Compared with patients with ≤ 2 bone metastases, patients with > 2 bone metastases had significantly higher t-psa and f-psa levels and significantly lower f/t ratio, with statistically significant differences(P < 0.05). With the increase of f/t ratio, the proportion of patients with bone metastasis decreases. Conclusion The bone metastasis of prostate cancer in the elderly was mainly in multiple lesions, mainly in the vertebral body and ribs. The combined SPECT with t-PSA, f-PSA and f/t can improve the diagnosis rate of bone metastasis in elderly prostate cancer.
