ExoSeal血管封堵器在肱动脉高位穿刺点止血中的临床应用
Clinical application of ExoSeal vascular closure device in the hemostasis of “high position” puncture point of brachial artery
【摘要】 目的评估ExoSeal血管封堵器在肱动脉"高位"穿刺点止血中应用的安全性和有效性。方法 53例肱动脉高位穿刺患者[女17例,男36例,平均年龄(75.7±8.9)岁],按不同止血方法分为ExoSeal血管封堵器止血组(ExoSeal组,n=37)和手按压止血组(MC组,n=16)。分析两组间止血时间、术后制动时间和围手术期并发症发生率。术后随访3~12个月,平均(9.8±2.9)个月。结果 ExoSeal组平均止血时间[(3.3±1.4) min]与MC组[(12.7±3.0) min]相比显著缩短(P<0.05),术后平均制动时间[(3.2±1.2) h]与MC组[(5.9±1.4) h]相比显著缩短(P<0.05)。围手术期ExoSeal组出现皮下血肿(<6 cm)1例,穿刺点渗血1例,并发症发生率为5.4%;MC组出现皮下血肿(<6 cm)1例,穿刺点处假性动脉瘤(瘤径3 mm)1例,穿刺点渗血1例,并发症发生率为18.8%。结论ExoSeal血管封堵器应用于肱动脉高位穿刺术后止血安全有效,可明显缩短穿刺点术后止血时间和制动时间。
【Abstract】 Objective To evaluate the safety and efficacy of ExoSeal vascular closure device in the hemostasis of "high position"puncture point of brachial artery. Methods A total of 53 patients receiving"high position"puncture of the brachial artery, including 17 females and 36 males with a mean age of(75.7±8.9)years, were divided, according to different hemostasis methods, into ExoSeal vascular closure device group(ExoSeal group,n=37) and manual compression group(MC group, n=16). The hemostasis time, postoperativebody immobilization time and the perioperative complication incidence were compared between the two groups. The patients were followed up for 3-12 monthswith an average of(9.8±2.9) months. Results the average hemostasis time in the ExoSeal group was(3.3±1.4) min, which was significantly shorter than(12.7±3.0) min in the MC group(P<0.05). The averagepostoperative body immobilization time in the ExoSeal group was(3.2±1.2) hours, which was remarkably shorter than(5.9±1.4) hours in the MC group(P<0.05). In the ExoSeal group the perioperative complications included subcutaneous hematoma(<6 cm, n=1) and errhysisat puncture point(n =1), the incidence of complicationswas 5.4%. In the MC group the perioperative complications included subcutaneous hematoma(<6 cm, n=1), pseudoaneurysm formation(diameter of 3 mm, n=1) at the puncture point, and errhysisat puncture point(n=1), the incidence of complicationswas 18.8%. Conclusion For the hemostasis of "high position"puncture point of brachial artery, ExoSeal vascular closure device issafe and effective, it can remarkably reduce the postoperative hemostasis time of puncture point as well as the body immobilization time.
