【摘要】 目的通过观察冠心病患者左室舒张功能与冠脉狭窄严重程度的相关性,探讨其在冠脉病变预判上的临床价值,为疾病的预防及治疗提供参考。方法收集拟行择期冠脉造影检查的可疑冠心病患者200名。根据冠脉造影结果分两组,其中CHD组150例,非CHD组50例。分析两组基本临床资料及常规超声下的左心房内径(LA)、左心室后壁厚度(LVPWT)、左心室射血分数(LVEF)、三尖瓣反流、舒张早期和晚期最大血流速度比(E/A)。结果两组患者的基本临床资料比较,CHD组与非CHD组的性别、年龄、吸烟及饮酒史、患糖尿病、患高血压、甘油三酯、胆固醇、低密度脂蛋白、高密度脂蛋白、血红蛋白、血清肌酐水平及LVEF、LVPWT差异均无统计学意义(P>0.05)。CHD组中LA、E/A比值、Gensini积分明显高于非CHD组,差异具有统计学意义(P<0.05)。同时,CHD组患者左室舒张功能指标中E/A比值与Gensini评分之间有独立相关关系(P<0.05)。结论左心室舒张功能减退程度与冠脉狭窄严重程度呈正相关,对冠脉病变程度的早期预判有意义。更多还原

【Abstract】 Objective To observe the correlation between left ventricular diastolic function and the severity of coronary stenosis in patients with coronary heart disease,and to explore its clinical value in predicting coronary artery disease,and to provide a reference for the prevention and treatment of this disease. Methods A total of 200 patients with suspected coronary heart disease who planned to undergo elective coronary angiography were collected. According to the results of coronary angiography,they were divided into two groups,including 150 cases in CHD group and 50 cases in non-CHD group.The basic clinical data and LA,LVPWT,LVEF,TR,and E/A ratios of the two groups were analyzed. Results The basic clinical data of the two groups of patients were compared with the differences in gender,age,smoking and drinking history,diabetes,hypertension,TC,TG,LDL,HDL,HB,Scr levels,and LVEF and LVPWT between the CHD group and the non-CHD group. None were statistically significant( P > 0. 05). The left atrial diameter,E/A ratio,and Gensini score in the CHD group were significantly higher than those in the non-CHD group,and the differences were statistically significant( P < 0. 05). At the same time,there was an independent correlation between E/A ratio and Gensini score in left ventricular diastolic function index in patients with CHD( P <0. 05). Conclusion The degree of left ventricular diastolic dysfunction is positively correlated with the severity of coronary stenosis,and it is meaningful for early prediction of the degree of coronary artery disease.更多还原
