【摘要】 目的探讨造血干细胞移植(hematopoietic stem cell transplantation,HSCT)术后患者疲乏、心理弹性与抑郁之间的相关性。方法 2019年6-12月,采用便利抽样法选取湖南省3所三级甲等医院血液内科门诊随访的HSCT术后2年内的160例患者为研究对象,采用一般资料调查表、癌症疲乏量表(cancer fatigue scale,CFS)、心理弹性量表(the connor-davidson resilience scale,CD-RISC)和抑郁自评量表(self-reting depression scale,SDS)对其进行调查。结果 HSCT术后患者的疲乏、心理弹性、抑郁严重度指数得分分别为(25.32±8.02)分、(62.06±14.80)分、(0.50±0.09)分;疲乏与抑郁呈正相关(r=0.598,P<0.01),疲乏与心理弹性呈负相关(r=-0.424,P<0.01),心理弹性与抑郁呈负相关(r=-0.633,P<0.01)。心理弹性在疲乏与抑郁间的部分中介效应显著(β=0.09,P<0.05),中介效应占总效应的10.98%。结论疲乏对HSCT术后患者抑郁既存在直接影响,也通过心理弹性的中介效应对患者抑郁发挥间接影响作用;血液科护理人员可从疲乏及心理弹性的角度对患者进行干预,以预防和减少HSCT术后患者抑郁的发生。更多还原

【Abstract】 Objective To investigate the relationship among fatigue,mental resilience and depression in patients after hematopoietic stem cell transplantation.Methods The general data questionnaire,cancer fatigue scale,the Connor-Davidson resilience scale and self-rating depression scale were used to investigate 160 patients who underwent hematopoietic stem cell transplantation 2 years after operation in three tertiary hospitals in Hunan Province.Results The scores of fatigue,mental resilience and depression severity were(25.32±8.02),(62.06±14.80) and(0.50±0.09),respectively.There was a moderate positive correlation between fatigue and depression(r=0.598,P<0.01),a moderate negative correlation between fatigue and resilience(r=-0.424,P<0.01),and a strong negative correlation between resilience and depression(r=0.633,P<0.01).The partial mediating effect of mental resilience between fatigue and depression was significant(β=0.09,P<0.05),and the mediating effect accounted for 10.98% of the total effect.Conclusions Fatigue has a direct effect on depression in patients with hematopoietic stem cell transplantation,but it also has an indirect effect on depression through the mediating effect of mental resilience.In order to prevent and reduce the occurrence of depression in patients with hematopoietic stem cell transplantation,nurses in the hematology department can intervene from the perspective of fatigue and mental resilience.更多还原
