无创辅助通气在新生儿呼吸窘迫综合征治疗中的应用进展
Application Progress of Non-invasive Assisted Ventilation in the Treatment of Neonatal Respiratory Distress Syndrome
【摘要】 在新生儿呼吸窘迫综合征(respiratory distress syndrome,RDS)的治疗中,有创辅助通气的不良后果,使得对无创辅助通气(non-invasive ventilation,NIV)模式的研究更加深入。NIV能显著改善早产儿RDS死亡率和减少使用有创辅助通气的不良后果。在不同的NIV模式中,经鼻间歇正压通气(nasal intermittent positive pressure ventilation,NIPPV)较经鼻持续正压通气(nasal continuous positive airway pressure,NCPAP)有显著的优势。与NCPAP相比,加热加湿高流量鼻导管(heated humidified high-flow nasal cannula,HHHFNC)同样具有优势。此外,RDS应用插管后给予表面活性物质后拔管,续贯予NIV,能有更高的抢救成功率。故该文对目前NIV的各种模式进行综述分析。
【Abstract】 In the treatment of neonatal respiratory distress syndrome(RDS), the adverse consequences of invasive assisted ventilation have made the research on non-invasive ventilation(NIV) mode more in-depth. NIV can significantly improve RDS mortality and reduce the adverse consequences of using invasive assisted ventilation in preterm infants. In different NIV modes, nasal intermittent positive pressure ventilation(NIPPV) has significant advantages over nasal continuous positive airway pressure(NCPAP). Compared with NCPAP, heated humidified high-flow nasal cannula(HHHFNC) also has advantages. In addition, after RDS application, after intubation, the surfactant was given after extubation, and NIV was continuously applied, which could have a higher rescue success rate. Therefore, this article reviews and analyzes the current NIV models.
