多b值DWI及ADC值定量分析在乳腺良恶性病变鉴别诊断中价值
Value of multiple b value DWI and quantitative analysis of ADC value in the differential diagnosis of benign and malignant breast lesions
【摘要】 目的研究3.0T MRI多b值DWl序列在乳腺良恶性病变中的诊断价值。方法收集复旦大学附属中山医院青浦分院2018年1月~至2019年12月60例女性乳腺疾病患者,其中良性病变38例,恶性病变22例,均进行多b值DWI扫描及增强扫描,获得标准表观扩散系数(ADCst0值,慢速表观扩散系数(ADCslow)值,快速表观扩散系数(ADCfast)值,并以病理检查结果为"金标准",分为良性及恶性两组,分析两组以上参数的差别。结果乳腺良性病灶的ADCst、ADCslow值均高于乳腺恶性病灶,差异有统计学意义(P<0.05)。乳腺良性病灶的ADCfast值低于乳腺恶性病灶,差异有统计学意义(P<0.05)。结论 ADCst、ADCslow、ADCfast值能够协助诊断乳腺良恶性病变,对乳腺良恶性病变的鉴别诊断具有重要的临床意义。
【Abstract】 Objective To explore the diagnostic value of multiple b value DWl sequence of 3.0 T MRI in benign and malignant breast lesions. Methods Sixty female patients with breast diseases from January 2018 to December 2019 were collected in our hospital, including 38 cases of benign lesions and 22 cases of malignant lesions, they were all examined by multiple b-value DWI scanning and enhanced scanning to calculate the value of standard apparent diffusion coefficient(ADCst), slow apparent diffusion coefficient(ADCslow) and fast apparent diffusion coefficient(ADCfast), they were divided into two groups of benign and malignant with pathological examination results as the "golden standard". The difference of the above parameters between two groups was analyzed, and P<0.05 was considered to be of statistically significance. Results The ADCst and ADCslow of benign breast lesions were both higher than those of malignant breast lesions, and the difference was statistically significant. ADCFAST of benign breast lesions was lower than that of malignant breast lesions with statistically significance. Conclusion ADCst, ADCslow and ADCfast values can be used to diagnose benign and malignant breast lesions, which has great clinical significance in the differentiation and diagnosis of benign and malignant breast lesions.
