脉络膜转移癌的眼部影像学特征分析

【摘要】 目的分析脉络膜转移癌的眼部影像学检查结果,为该病的正确诊断提供依据。方法回顾性分析7例(8只眼)确诊为脉络膜转移癌患者的影像学资料,包括眼底彩照、眼部超声,荧光素眼底血管造影(fluorescein fundus angiography,FFA),吲哚菁绿血管造影(indocyanine agiography,ICGA)及光学相干断层扫描(optical coherence tomography,OCT),总结其影像学特征。结果典型病例眼底彩照可见后极部扁平状黄白色实性隆起灶;眼部超声表现为扁平隆起的等回声团;FFA表现为病灶处早期低荧光,晚期扩大融合,呈现边界清晰的强荧光灶;ICGA早期、中期瘤体均表现为低荧光,晚期荧光增强;OCT显示病灶处视网膜色素上皮层粗糙伴隆起,视网膜神经上皮层不同程度浅脱离。结论脉络膜转移癌有一定的特征性表现,可为临床诊断和鉴别诊断提供依据。

【Abstract】 Objective To analysis choroid metastatic carcinoma eye imaging examination results, provide the basis for the correct diagnosis of the disease. Methods Seven cases(8 eyes) diagnosed with choroid metastatic carcinoma imaging data were retrospectively analyzed, including fundus color picture, ocular ultrasound, fluorescein fundus angiography(FFA), indocyanine green angiography(ICGA) and optical coherence tomography(OCT), the imaging features were summarized. Results Fundus color picture of typical cases showed visible flat yellowish-white solid uplift in pole of the eye; Ocular ultrasound showed flat uplift hypoechoic mass; FFA characterized by low fluorescence in early stage of lesion, fluorescence expanded and fused in the late stage, presented as boundaries clearly strong fluorescence lesion; In the early, middle stage of ICGA, the lesion characterized by low fluorescence, fluorescence enhancement in the late stage; OCT showed the retinal pigment epithelium at coarse and uplift, the retinal neural epithelium separated from different level. Conclusion Choroid metastatic carcinoma has certain characteristics, which chould provide the basis for clinical diagnosis and differential diagnosis.
