【摘要】 目的探讨椎体血管瘤（VH）的影像学表现及鉴别要点。方法回顾性分析2016年1月至2019年1月禹州市人民医院28例经手术、病理确诊为VH的患者的临床资料。所有患者均接受X线检查,20例接受CT检查,24例接受MRI检查。对3种检查的影像学表现进行对比分析。结果 28例患者中单发者20例,多发者8例,共36处病灶,颈椎占16.7%（6/36）,胸椎占52.8%（19/36）,腰椎占30.5%（11/36）。X线检出16处病灶,显示骨小梁排列稀疏,椎体呈骨质疏松样改变,10处表现为"栅栏征"。经CT检查,共27处病灶均获显示,表现为纵向骨小梁间隙增宽,呈"网眼状"。经MRI检查,共26处病灶均获显示,其中16处为稍短T₁、长T₂信号,10处为长T₁、长T₂信号（其中4处T₂高信号病灶中间可见呈低信号的增粗骨小梁）。结论 VH的X线、CT、MRI表现具有一定特征,其中CT是主要检查手段,可显示骨质改变情况,表现为"栅栏征"或"网眼状"等,MRI对明确VH的分型和压迫性VH对椎管、椎间孔的挤压程度具有优势。更多还原

【Abstract】 Objective To explore the imaging features and identification points of vertebral hemangioma（VH）.Methods The clinical data of 28 patients who were diagnosed as VH by surgery and pathology from January 2016 to January 2019 in Yuzhou City People’s Hospital were retrospectively analyzed. All patients underwent X-ray examination, 20 patients underwent CT examination, and 24 patients underwent MRI examination. The imaging performances of the 3 examinations were compared and analyzed.Results Among 28 patients, 20 were single and 8 were multiple. There were 36 lesions in total, 16.7%（6/36） in cervical spine, 52.8%（19/36） in thoracic spine, and 30.5%（11/36） in lumbar spine. X-rays detected 16 lesions, showing that the trabecular bones were sparsely arranged, the vertebral body showed osteoporotic changes, and 10 showed "palisade sign". In CT examination, a total of 27 lesions were revealed, showing a widening of the longitudinal trabecular bone space and "mesh shape". After MRI examination, a total of 26 lesions were displayed, of which 16 were slightly shorter T₁ and long T₂ signals, and 10 were longer T₁ and long T₂ signals.Conclusion The X-ray, CT, and MRI manifestations of VH have certain characteristics. Among them, CT is the main examination method, which can show the changes of bone quality, such as "fence sign" or "reticulate". MRI has advantages in determining the classification of VH and the degree of compression of vertebral canal and intervertebral foramen by compressive VH.更多还原
