【摘要】 目的探讨集束化护理策略应用于ECMO辅助支持治疗患者的临床效果。方法选择2017年3月至2019年4月于ICU接受ECMO支持治疗的患者120例,按照组间基本特征具有可比性的原则分成为对照组和观察组。对照组采用常规ABCDE集束化护理方案,观察组在对照组基础上采用集束化策略。比较两组血流动力学指标、机械通气时间、ICU入住时间、镇静药用量、ECMO辅助时间和谵妄发生情况。结果干预后,观察组中心静脉压、心率低于对照组(P <0.05),平均动脉血压、氧和指数高于对照组(P <0.05);观察组机械通气时间、ICU入住时间和ECMO辅助时间短于对照组(P <0.05),观察组镇静药用量少于对照组(P <0.05);观察组谵妄发生率低于对照组(P <0.05)。结论集束化护理策略应用于ECMO辅助患者中,可改善患者的血流动力学指标,降低镇痛镇静药用量和谵妄发生率,缩短患者治疗恢复时间,值得临床推广应用。更多还原

【Abstract】 Objective To explore the clinical effect of clustering nursing strategy applied to ECMO-assisted patients. Methods To select 120 cases of patients who received ECMO supportive treatment in the ICU from March 2017 to April 2019 as the study objects,they were divided into a control group and an observation group according to the principle of comparability of basic characteristics between groups. The control group adopted the conventional ABCDE clustering nursing plan,and the observation group adopted the clustering strategy on the basis of the control group to compare the hemodynamic indexes,mechanical ventilation time,ICU stay time,dosage of sedative,assistance time of ECMO and the occurrence of delirium between the two groups. Results After intervention,the observation group’s central venous pressure and heart rate were lower than those of the control group( P < 0. 05),and the average arterial blood pressure,oxygen and index were higher than those of the control group( P < 0. 05). The mechanical ventilation time,ICU stay time and assistance time of ECMO in the observation group were shorter than those in the control group( P < 0. 05). The dosage of sedative in the observation group was less than that in the control group( P < 0. 05). The incidence of delirium in the observation group was lower than that in the control group( P < 0. 05). Conclusion The clustering nursing strategy applied to ECMO-assisted patients can improve the patients’ hemodynamic indicators,reduce the dosage of analgesics and sedatives and the incidence of delirium,and shorten the treatment recovery time of patients.
