巨细胞病毒肝损害患儿外周血IL-17、IL-35与Th17/Treg的表达及临床意义
Expression and clinical significance of IL-17,IL-35 and Th17/Treg in peripheral blood of children with hepatic lesion induced by HCMV
【摘要】 目的:探讨巨细胞病毒(HCMV)肝损害患儿外周血中IL-17、IL-35、辅助性T细胞(Th17)细胞及CD4+CD25+17调节性T细胞(CD4+CD25+regulatory T cells,Treg)水平及临床意义。方法:选择巨细胞病毒感染合并有肝损害患儿20例作为肝损害组,选取虽然存在巨细胞病毒感染但没有肝脏损害患儿18例作为无肝损害组,同时选择20例健康儿童作为健康组。采用荧光免疫PCR方法检测3组儿童尿HCMV-DNA含量,ELISA试剂盒检测儿童外周血IL-17、IL-35含量,流式细胞仪检测Th17/Treg细胞比例,并同时测定谷丙转氨酶(ALT)含量。结果:肝损害组外周血ALT明显高于健康组和无肝损害组(P均<0.05);肝损害组患儿外周血IL-17含量、Th17细胞比例及Th17/Treg高于健康组和无肝损害组(P <0.05),IL-35含量和Treg细胞比例低于健康组和无肝损害组(P <0.05)。结论:IL-17、IL-35与巨细胞病毒感染引起的肝损害关系密切。在巨细胞病毒感染的患儿中,Th17/Treg的比例平衡对保持内环境的稳定和免疫功能的恢复发挥了重要用。
【Abstract】 Objective: To explore the levels of IL-17,IL-35 and Th17/Treg in peripheral blood of children with HCMV combined with hepatic lesion so as to provide references for the clinical diagnosis and treatment. Methods: 20 children with HCMV combined with hepatic lesion were chosen as a hepatic lesion group,18 children with HCMV but without hepatic lesion were selected as a non-hepatic lesion group,and another 20 healthy children were recruited as a healthy group. HCMV-DNA content in urine( by fluorescence immuno PCR),IL-17 and IL-35 in peripheral blood( by ELISA kit),rate of Th17/Treg( by flow cytometry) and ALT of the three groups were detected simultaneously. Results: The index of ALT of the hepatic lesion group was significantly higher than that of the healthy group or the non-hepatic lesion group( all P < 0. 05). The IL-17 content,the rate of IL-17 and the Th17/Treg level of the peripheral blood in the hepatic lesion group were higher than those of the healthy group or the non-hepatic lesion group( P < 0. 05). The IL-35 content and the rate of Treg cells were lower than those of the healthy group or the non-hepatic lesion group( P < 0. 05). Conclusion: IL-17 and IL-35 are closely related with hepatic lesion induced by HCMV. In children with HCMV infection,the rate balance of Th17/Treg contributes significantly to the stability of the internal environment and the recovery of immune function.
