磁共振动态增强定量参数对克罗恩病病情的评估效果
Evaluated effect of dynamic enhanced quantitative parameters of magnetic resonance on Crohn’s disease
【摘要】 目的:探究磁共振动态(DCE-MRI)增强定量参数与容积转运常数(Ktrans)对克罗恩病(CD)病情的评估效果。方法:选取某院接受治疗的50例克罗恩病患者,对患者分别进行MRI、DCE-MEI扫描和Ktrans值测量,同时收集患者的HBI和ADC等各项资料。将50例患者根据HBI水平分为重度活动组10例、轻中度活动组22例和静止组18例,对3组患者的Ktrans、ADC进行比较,分析活动期和慢性期CD患者MRI表现及Ktrans、ADC、HBI的相关性。结果:3组患者Ktrans值、ADC值比较,差异有统计学意义(P<0.05);MRI诊断CD的结果显示:假阳性患者5例,假阴性患者23例,敏感度为66.2%,特异度为84.4%,阳性预测值为90.0%,阴性预测值为54.0%;Spearman和Pearson相关性分析显示:50例CD患者的Ktrans与HBI呈现正相关,与ADC呈现负相关,血清ADC与Ktrans呈现负相关(P均<0.05)。结论:MRI定量参数Ktrans在反应CD活动状态方面有重要作用,具有一定的临床应用价值。
【Abstract】 Objective: To investigate the effect of dynamic-contrast enhanced magnetic resonance imaging(DCE-MRI) on quantitative assessment of Crohn’s disease(CD).Methods: 50 patients with CD were selected for routine MRI and DCE-MRI scanning. Ktrans was measured and the patients’ HBI and C-reactive protein(CRP) and other general information were collected. The 50 patients were divided into a severe activity group(10 cases), a mild to moderate group(22 cases) and a static group(18 cases) according to HBI level. The Ktrans and ADC of the three groups were compared to analyze the MRI findings of active and chronic CD patients and correlations between Ktrans, ADC and HBI.Results: In comparison of Ktrans and ADC between the three groups of patients, the difference was statistically significant(P<0.05). The findings of MRI diagnosed CD indicated that there were 5 false positive cases, 23 false negative cases, 45 true positive cases and 27 true negative cases.The result of DCE-MRI assessmerit revealed that the sensitivity was 66.2%, the specificity 84.4%, the positive predictive value 90.0%, and the negative predictive value 54.0%. Correlation analysis of Spearman and Pearson showed that Ktrans of the 50 CD patients were positively correlated with HBI and negatively correlated with ADC and serum ADC indicated negative correlation with Ktrans(P<0.05).Conclusion: Quantitative parameter Ktrans of MRI plays an important role in CD activity, and has a high clinical value.
