【摘要】 目的探讨自拟舒筋活血方熬制黑膏药治疗腰椎间盘突出症（LDH）的临床疗效。方法选择气滞血瘀型LDH患者100例随机分为对照组和观察组,每组50例。在常规治疗的基础上,对照组采用奇正消痛贴,2 d/贴,观察组采用自拟舒筋活血方黑膏药,2 d/贴。2组患者均贴于突出腰椎及上下各一节夹脊穴,配合环跳穴等穴位。10 d为1个疗程,共治疗2个疗程。比较2组患者临床疗效、疼痛情况以及生活质量。结果观察组临床疗效显著优于对照组（Z=3.8,P<0.05）。2组患者ODI评分（The Oswestry Disability Index）随治疗时间的延长显著下降(F_时间=3 718.5,P<0.05),观察组ODI评分下降幅度显著高于对照组(F_交互=36.6,P<0.05),观察组ODI评分整体水平显著低于对照组(F_组间=8.6,P<0.05)。2组患者疼痛分级指数（pain rating index,PRI）随治疗时间的延长显著下降(F_时间=2 077.3,P<0.05),观察组PRI下降幅度显著高于对照组(F_交互=28.3,P<0.05),观察组PRI整体水平显著低于对照组(F_组间=6.8,P<0.05)。2组患者现时疼痛强度（present pain intensity,PPI）随治疗时间的延长显著降低(F_时间=1 931.3,P<0.05),观察组PPI降低幅度显著高于对照组(F_交互=6.9,P<0.05),观察组PPI整体水平显著低于对照组(F_组间=5.4,P<0.05)。2组患者视觉模拟评分（visual analogue scale,VAS）随治疗时间的延长显著下降(F_时间=809.4,P<0.05),观察组VAS评分下降幅度显著高于对照组(F_交互=4.4,P<0.05),观察组VAS评分整体水平显著低于对照组(F_组间=3.8,P<0.05)。治疗后观察组生活质量评价量表（short form 36-item health survey,SF-36）各项评分均显著高于对照组（t分别为2.6,2.7,2.3,2.0,2.3,2.4,2.1,2.6,P<0.05）。结论自拟舒筋活血方熬制黑膏药,对于气滞血瘀型LDH具有较好的临床疗效,能够明显缓解患者的腰痛症状,改善腰部活动能力。更多还原

【Abstract】 Objective To investigate the therapeutic effects of self-made black plaster according to ShujinHuoxue prescription on lumbar disc herniation(LDH).Methods One hundred patients with LDH were randomly divided into control group and observation group,with 50 cases in each group.On the basis of routine treatment,the patients in control group were treated by Qizhengxiaotong plaster,2 d/patch,and the patients in observation group were treated by self-made black plaster,2 d/patch.The plasters in both groups were attached to the protruding lumbar spine and the upper and lower segments in Jiaji acupoint and Huantiao acupoint,with 10 days as a treatment course,a total of 2 courses.The clinical curative effects,pain degree and life quality of patients were observed and compared between the two groups.Results The therapeutic effects in observation group were superiot to those in control group(P<0.05).With the time going on the Oswestry Disability Index(ODI) scores in both groups were decreased significantly(P<0.05),moreover,the decrease degree in observation group was more significant than that in control group(P<0.05). The overall levels of ODI score in observation group were significantly lower than those in control group(P<0.05). The pain rating index(PRI) in both groups was decreased significantly(P<0.05),moreover,the decrease degree in observation group was more significant than that in control group(P<0.05).In addition the overall levels of PRI score in observation group were significantly lower than those in control group(P<0.05). The Visual analogue scale(VAS) scores in both groups were decreased significantly(P<0.05), and the decrease degree in observation group was more significant than that in control group(P<0.05).The overall levels of VAS in observation group were significantly lower than those in control group(P<0.05).After treatment,the scores of Short Form 36-item Health Survey(SF-36) in observation group were significantly higher than those in control group(P<0.05).Conclusion The self-made black plaster according to ShujinHuoxue prescription has better therapeutic effects on LDH,which can obviously relieve the pain,improve the activity ability of low back of patients.更多还原
