【摘要】 目的研究彩色多普勒超声(CDU)联合超声弹性成像(UE)、超声高精细血流成像技术(Fine-Flow)对小乳癌腋窝淋巴结转移的诊断价值。方法选取2015年1月至2018年1月收治的74例小乳癌患者为研究对象,分别采用单纯CDU检查、CDU+UE+Fine-Flow检查,并以病理检查结果为金标准,观察单纯CDU检查、CDU+UE+Fine-Flow检查诊断小乳癌腋窝淋巴结性质的灵敏度、特异度、准确度、阳性预测值、阴性预测值及腋窝淋巴结转移者与未转移者超声弹性成像评分、弹性应变率。结果病理检查结果显示,74例小乳癌患者诊断为腋窝淋巴结转移者64例,未转移者10例。CDU+UE+Fine-Flow诊断小乳腺腋窝淋巴结性质的灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于单独CDU检查,差异有统计学意义(P<0.05)。腋窝淋巴结转移者弹性评分主要集中在3～4分,腋窝淋巴结未转移者弹性评分主要集中在1分,差异有统计学意义(P<0.05);腋窝淋巴结转移者与未转移者弹性应变率分别为(2.70±0.45)%、(1.51±0.23)%,腋窝淋巴结转移者弹性应变率高于未转移者,差异有统计学意义(P<0.05)。结论 CDU联合UE、Fine-Flow对小乳癌腋窝淋巴结性质的诊断价值高于单纯CDU检查,具有良好的应用前景。更多还原

【Abstract】 Objective To investigate the diagnostic value of ultracolor Doppler ultrasonography(CDU) combined with magnetic resonance imaging(UE) and fine-flow ultrasonography(Fine-Flow) in diagnosis of axillary lymph node metastasis of small breast cancer.Methods A total of 74 patients with small breast cancer who were admitted and treated in our hospital from January 2015 to January 2018 were enrolled in the study,who were examined by simple CDU or CDU+UE+Fine-Flow.Taking the pathological examination results as gold standard to evaluate the sensitivity,specificity,accuracy,positive predictive value,negative predictive value,the mean ultrasound score and elastic strain rate of metastatic axillary lymph nodes and non-metastatic axillary lymph nodes by the two examination schemes in diagnosis of small breast cancer.Results The pathological examination results showed that among the 74 patients with small breast cancer,there were 64 cases of axillary lymph node metastasis and 10 cases of non-axillary lymph node metastasis. The sensitivity,specificity,accuracy,positive predictive value and negative predictive value by CDU+UE+Fine-Flow in diagnosis of small breast lumps were significantly higher than those by single CDU examination(P<0.05).The mean ultrasound scores of metastatic axillary lymph nodes were mostly in 3～4 points,which in non-metastatic axillary lymph nodes were mostly in 1 point,there were significant differences between them(P<0.05).Moreover the elastic strain rates of metastatic axillary lymph nodes and non-metastatic axillary lymph nodes were(2.70±0.45)% and(1.51±0.23)%,respectively, there were significant differences between them(P<0.05).Conclusion As compared with single CDU examination,the CDU combine with UE and Fine-Flow have higher diagnostic value in diagnosis of axillary lymph node metastasis of small breast cancer,which has better clinical application prospect.更多还原
