ESR1、MYOD1和hTERT基因启动子DNA甲基化与宫颈癌放化疗敏感性的关系
Relationship between DNA methylation of ESR1,MYOD1,hTERT gene promoter and sensitivity to radiotherapy and chemotherapy in patients with cervical cancer
【摘要】 目的评估雌激素受体1(ESR1)、肌源性分化1(MYOD1)和人端粒酶逆转录酶(hTERT)基因启动子甲基化谱和相应的转录本表达情况,以确定能够预测化学放射治疗患者的反应的基因。方法纳入2015年1月至2018年11月102例FIGOⅡB/Ⅲ期宫颈癌的病理诊断,取常规宫颈活检进行组织病理学诊断,随后进行化学放射治疗。治疗方案完成后随访,患者分为化学放射敏感性和化学放射耐受性2组,分别用TaqMan法和定量实时PCR法检测组织标本中基因启动子甲基化和基因表达。结果 ESR1、BRCA1、RASSF1A、MLH1、MYOD1和hTERT基因甲基化频率在40%~70%。单因素和层次聚类分析表明,MYOD1、ESR 1和hTERT基因启动子甲基化可以预测化疗反应。非甲基化MYOD1、非甲基化ESR 1和甲基化hTERT启动子的一种模式,以及较低的ESR 1转录本水平。结论包括MYOD1、ESR 1和hTERT在内的三个基因的甲基化谱可能有助于预测接受标准化学放射治疗的浸润性宫颈癌患者的反应。
【Abstract】 Objective To investigate the methylation status of ESR1,MYOD1 and hTERT gene promoters and the expressions of corresponding transcripts to identify the genes that can predict the response of patients to chemotherapy.Methods A toatl of 102 patients with FIGO Ⅱ B/Ⅲ cervical carcinoma who were treated in our hospital from from 2015 to November 2018 were enrolled in the study.The routine cervical biopsy was used for histopathological diagnosis,then chemotherapy was performed.The patients were divided into two groups:chemosensitivity group and chemoradioresistance group.The promoter methylation and gene expression in tissue samples were detected by TaqMan assay and quantitative real time PCR method,respectively.Results The methylation frequency of ESR1, BRCA1, RASSF1 A, MLH1, MYOD1 and hTERT genes ranged from 40% to 70%.The univariate and hierarchical cluster analysis showed that gene promoter methylation of MYOD1,ESR1 and hTERT could predict the chemoradiation response.A pattern of unmethylated MYOD1,unmethylated ESR1 and methylated hTERT promoter as well as lower ESR1 transcript levels could predict the chemoradiation resistance of patients.Conclusion The methylation spectrum of MYOD1,ESR1 and hTERT may be useful to predict the response of patients with invasive cervical carcinoma undergoing standard chemoradiation therapy.
