【摘要】 目的观察百令胶囊联合穴位贴敷治疗溃疡性结肠炎的疗效及其对趋化因子配体12(CXCL12)/趋化因子受体4(CXCR4)信号轴的影响。方法选择2015年1月～2016年12月在我院就诊的溃疡性结肠炎的患者90例,根据随机数字法将患者分为观察组和对照组,每组各45例。对照组予以常规西药治疗,观察组在对照组的基础上予以百令胶囊联合穴位贴敷治疗。比较两组治疗后疗效和不良反应发生率,及治疗前后疾病活动指数评分,肠镜积分,病理组织积分,免疫球蛋白(Ig)A,IgM,IgG,CXCL12,CXCR4和血管内皮生长因子(VEGF)水平的变化。结果观察组的总有效率为97.78%明显高于对照组的80.00%,差异具有显著性差异(χ~2=5.513,P<0.05)。观察组和对照组的不良反应发生率差异无统计学意义(χ~2=0.320,P>0.05)。两组治疗前疾病活动指数评分,肠镜积分,病理组织积分,IgA,IgM,IgG,CXCL12,CXCR4和VEGF水平差异无统计学意义(P>0.05),治疗后两组的疾病活动指数评分,肠镜积分,病理组织积分,IgA,IgG,CXCL12,CXCR4和VEGF水平均较治疗前明显降低(P<0.01),而观察组与对照组比较降低水平更为明显(P<0.01),而两组治疗前后IgM水平差异无统计学意义(P>0.05)。结论百令胶囊联合穴位贴敷治疗溃疡性结肠炎的疗效显著,能够明显改善症状和体征,其机理可能与抑制CXCL12/CXCR4信号轴有关。更多还原

【Abstract】 OBJECTIVE To observe the efficacy of BaiLing capsule combined with acupoint application in the treatment of ulcerative colitis and its impact on the signal axis of chemokine ligand 12(CXCL12)/chemokine receptor 4(CXCR4).METHODS 90 patients with ulcerative colitis who were admitted to our hospital from January 2015 to December 2016,were divided into observation group and control group according to random number method,with 45 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with bailing capsule combined with acupoint application on the basis of the control group.The efficacy and adverse reaction rates was observed in the two groups after treatment,and the disease activity index scores,colonoscopy scores,pathological tissue scores,immunoglobulin(Ig) A,IgM,IgG,CXCL12,CXCR4 and vascular endothelial growth factor(VEGF) were also observed in two groups before and after treatment.RESULTS The total effective rate was 97.78% in the observation group,which was significantly higher than 80.00%in the control group(χ~2=5.513,P<0.05).There was no significant difference in the incidence of adverse reactions between the observation group and control group(χ~2=0.320,P>0.05).There were no significant differences in the disease activity index scores,colonoscopy scores,pathological tissue scores,IgA,IgM,IgG,CXCL12,CXCR4 and VEGF between the two groups(P>0.05).The colonoscopy scores,pathological tissue scores,IgA,IgG,CXCL12,CXCR4 and VEGF levels were significantly lower than those before treatment(P<0.01),while the magnitude of decrease in observation group was significantly lower compared with the control group(P<0.01).There was no significant difference in IgM levels between the two groups or before and after treatment(P>0.05).CONCLUSION BaiLing capsule combined with acupoint application is more obvious in the treatment of ulcerative colitis,which can significantly improve symptoms and signs,its mechanism may be related to the inhibition of CXCL12/CXCR4 signal axis.更多还原
