家庭干预对癌症患者及其配偶生活质量影响的Meta分析
Meta-analysis of the effect of family interventions on quality of life in cancer patients and their spouses
【摘要】 目的评估家庭干预对癌症患者及其配偶生活质量的效果。方法选取PubMed、Cochrane Library、Embase、CBM、中国知网、万方数据库及OVID数据平台从1980年1月至2020年5月公开发表关于癌症患者及其配偶同步家庭干预的随机对照试验研究(RCT)文献进行系统回顾。RevMan 5.3软件用于数据合并分析,采用固定或随机效应模型计算,同时按随访时间0~3个月、3.1~6个月、>6个月进行亚组分析。结果最终纳入9篇符合条件的文献,包括780对受试者。结果表明,癌症患者及其配偶同步的家庭干预组在随访6个月后患者生活质量总分相比对照组得分提高[SMD=0.60,95%CI=(0.08,1.11),P<0.01],配偶的生活质量生理健康在随访0~3个月[SMD=0.54,95%CI=(0.28,0.79),P<0.01]及>6个月[SMD=0.30,95%CI=(0.12,0.48),P<0.01]相比对照组均显著提高,配偶心理健康评分只在随访6个月后高于对照组[SMD=0.73,95%CI=(0.14,1.31),P<0.01]。结论癌症患者及其配偶同步的家庭干预可以同时提高其生活质量,但应在随访阶段加强针对患者生活质量和配偶生理和心理健康的干预强度以及注意其所产生的延迟效应。
【Abstract】 Objective To assess the impact of family interventions on the quality of life(QOL) of cancer patients and their spouses. Methods We conducted a systematic review and meta-analysis of randomized controlled studies(RCTs) published between Jan.1980 and May 2020 of family interventions conducted with both cancer patients and their spouses published in PubMed, Cochrane Library, Embase, CBM, China knowledge Network, Wanfang databases,and OVID data Platform. The combined effect size was calculated by fixed or random effect model, and subgroup analysis was carried out according to follow-up time of 0-3 months, 3.1-6 months, >6 months. Results A total of 780 pairs of subjects were included in 9 standard literatures. Meta-analysis showed that the total score of patients’ QOL in simultaneous family interventions of cancer patients and their spouses after 6 months of follow-up was higher than that of control group(SMD=0.60, 95%CI= [0.08,1.11], P<0.01). The quality of life and physical health of the spouses were significantly improved at 0-3 follow-up(SMD=0.54, 95%CI= [0.28,0.79], P<0.01) and >6 months(SMD=0.30, 95%CI= [0.12,0.48], P<0.01) compared with the control group. Spouses mental health score was higher than control group only 6 months after follow-up(SMD=0.73, 95% CI=[0.14,1.31], P<0.01). Conclusion Simultaneous family intervention of cancer patients and their spouses can improve the QOL of cancer patients and their spouses at the same time, but the intensity of interventions for patients’ QOL and their spouse’s physical health should be strengthened at the follow-up stage and their delayed effects should be paid attention to.
