不同亚型多囊卵巢综合征患者血清S100钙结合蛋白A12和高迁移率族蛋白B1水平及其临床意义

【摘要】 目的探讨不同亚型的多囊卵巢综合征(PCOS)患者血清S100钙结合蛋白A12(S100A12)和高迁移率族蛋白B1(HMGB1)水平及其临床意义。方法纳入40例健康女性(正常对照组)和94例PCOS患者,按照体质量指数(BMI)将PCOS患者分为高BMI-PCOS组(50例)和正常BMI-PCOS组(44例),按睾酮水平将PCOS患者分为高睾酮水平-PCOS组(46例)和正常睾酮水平-PCOS组(48例)。比较各亚型PCOS患者及正常对照组之间的血沉、白细胞计数、血清S100A12和HMGB1水平,并分析血清S100A12、HMGB1水平与BMI和睾酮水平的相关性。结果高BMI-PCOS组血沉、白细胞计数、血清S100A12和HMGB1水平均高于正常BMI-PCOS组和正常对照组(均P<0.05)。高睾酮水平-PCOS组的血清S100A12水平高于正常睾酮水平-PCOS组和正常对照组,且白细胞计数和血清HMGB1水平均高于正常对照组(均P<0.05)。PCOS患者血清HMGB1、S100A12水平均与BMI呈正相关(均P<0.05),但与睾酮均无相关性(均P>0.05);正常对照组血清HMGB1、S100A12水平与BMI或睾酮均无相关性(均P>0.05)。结论肥胖型PCOS患者的常规炎症指标及血清S100A12、HMGB1水平均较非肥胖PCOS患者及健康女性升高。PCOS患者血清HMGB1和S100A12水平均与BMI呈正相关,S100A12与HMGB1可能是反映PCOS患者慢性低度炎症的新指标。高雄激素水平的PCOS患者血清S100A12亦升高,但S100A12、HMGB1与PCOS患者的高雄激素水平的相关性有待进一步研究。

【Abstract】 Objective To investigate serum S100 calcium-binding protein A12(S100 A12) and high mobility group box 1 protein(HMGB1) levels and their clinical significance in patients with different subtypes of polycystic ovary syndrome(PCOS). Methods Forty healthy women(normal control group) and 94 PCOS patients were enrolled. The PCOS patients were divided into high body mass index(BMI)-PCOS group(n=50) and normal BMI-PCOS group(n=44) according BMI, and were divided into high testosterone level-PCOS group(n=46) and normal testosterone level-PCOS group(n=48) according to testosterone level. The erythrocyte sedimentation rate(ESR), WBC count, and serum S100 A12 and HMGB1 levels were compared between patients with different subtypes of PCOS and the normal control group. The correlation of serum S100 A12 and HMGB1 levels with BMI or testosterone level was analyzed.Results The ESR, WBC count, and serum S100 A12 and HMGB1 levels in the high BMI-PCOS group were higher than those in the normal BMI-PCOS group or normal control group(all P<0.05). The high testosterone level-PCOS group yielded a higher serum S100 A12 level as compared with the normal testosterone level-PCOS group or normal control group, and exhibited increased WBC count and serum HMGB1 level as compared with the normal control group(all P<0.05). Serum HMGB1 and S100 A12 levels positively correlated with BMI(all P<0.05), but did not correlate with testosterone(all P>0.05) in PCOS patients; serum HMGB1 or S100 A12 level did not correlate with BMI or testosterone in the normal control group(all P>0.05).Conclusion The routine inflammation indicators as well as serum S100 A12 and HMGB1 levels are all increased in obese PCOS patients as compared with PCOS patients with normal BMI and healthy women.Serum HMGB1 and S100 A12 levels positively correlate with BMI in PCOS patients, and S100 A12 and HMGB1 may be new indicators of low-grade chronic inflammation in PCOS patients. Serum S100 A12 is also increased in PCOS patients with elevated androgen level, but the correlation of S100 A12 or HMGB1 with elevated androgen level of PCOS patients still needs further study.
