中药联合阿奇霉素治疗对支气管扩张稳定期患者肺功能及血清炎症因子水平的影响
Effects of Chinese medicine combined with azithromycin therapy on pulmonary function and serum inflammatory factors levels in patients with stable bronchiectasis
【摘要】 目的观察自拟中药方联合阿奇霉素治疗对支气管扩张稳定期患者肺功能及血清炎症因子水平的影响。方法将122例支气管扩张稳定期患者随机分为研究组62例和对照组60例,分别给予自拟中药方联合阿奇霉素治疗和单用阿奇霉素治疗,疗程均为3个月。观察两组患者治疗前后肺功能1 s用力呼气容积(FEV1)和FEV1与用力肺活量的比值(FEV1/FVC),以及血清IgE、白细胞介素(IL)-4、IL-17水平的变化。结果治疗后研究组患者FEV1、FEV1/FVC比值高于治疗前及对照组(均P<0.05);治疗后两组患者IgE、IL-4、IL-17水平均低于治疗前,并且研究组均低于对照组(均P<0.05);随访6个月,研究组支气管扩张再次发作率低于对照组,再次发作间隔时间长于对照组(均P<0.05)。结论自拟中药方联合阿奇霉素治疗支气管扩张稳定期患者可有效地降低患者血清炎症因子水平,调节免疫功能,改善肺功能,并减少复发。
【Abstract】 Objective To observe the effects of self-made Chinese medicine prescription combined with azithromycin therapy on pulmonary function and serum inflammatory factors levels in patients with stable bronchiectasis. Methods A total of 122 patients with stable bronchiectasis were randomly divided into study group(n=62) and control group(n=60). The study group was treated with self-made Chinese medicine prescription combined with azithromycin, the control group with azithromycin alone, and the course of treatment was three months in both groups. The related indices were observed in the two groups before and after treatment, including forced expiratory volume in one second(FEV1), FEV1 to forced vital capacity(FEV1/FVC) ratio, as well as changes in serum levels of IgE, interleukin(IL)-4 and IL-17. Results After treatment, FEV1 and FEV1/FVC ratio in the study group were higher than those before treatment and the controls(all P<0.05); after treatment, the levels of IgE, IL-4 and IL-17 were lower than the levels before treatment in both groups, and the study group had lower levels than the control group(all P<0.05); the follow-up lasted for six months, the study group exhibited a lower recurrence rate of bronchiectasis and a longer recurrence interval as compared with the control group(all P<0.05). Conclusion The self-made Chinese medicine prescription combined with azithromycin therapy can effectively reduce serum inflammatory factors levels, modulate immune function, improve pulmonary function and reduce recurrence in patients with stable bronchiectasis.
