新生儿发生宫内感染性肺炎的危险因素

【摘要】 目的分析新生儿发生宫内感染性肺炎的危险因素。方法回顾性分析86例宫内感染性肺炎的足月新生儿(肺炎组)和129例非感染性疾病足月儿(对照组)的临床资料。收集两组患儿性别、分娩方式、羊水污染、胎膜早破、母亲产前发热(>37.8℃)、母亲合并妊娠期糖尿病(GDM),以及母亲分娩当天血白细胞计数、C反应蛋白、降钙素原等资料,分析新生儿发生宫内感染性肺炎的危险因素。结果胎膜早破以及母亲产前发热、合并GDM、血清降钙素原升高均是新生儿发生宫内感染性肺炎的独立危险因素(均P<0.05)。结论新生儿宫内感染性肺炎受多种因素的影响,对存在胎膜早破以及母亲产前发热、合并GDM、血清降钙素原升高等高危因素的新生儿应进行密切观察,以降低宫内感染性肺炎的发生风险。

【Abstract】 Objective To analyze the risk factors for intrauterine infectious pneumonia in neonates. Methods The clinical data of 86 full-term neonates with intrauterine infectious pneumonia(pneumonia group) and 129 full-term neonates with non-infectious diseases(control group) were analyzed retrospectively. The relevant data of the two groups were collected, including neonate gender, delivery approach, meconium-stained amniotic fluid, premature rupture of membranes, maternal antepartum fever(>37.8℃), maternal complication of gestational diabetes mellitus(GDM), as well as maternal WBC count, C-reactive protein and procalcitonin on the day of delivery, and the risk factors for intrauterine infectious pneumonia in neonates were analyzed.Results Premature rupture of membranes, and maternal antepartum fever, complication of GDM, and elevation of serum procalcitonin were all the risk factors for intrauterine infectious pneumonia in neonates(all P<0.05). Conclusion Neonatal intrauterine infectious pneumonia is affected by multiple factors, and close monitoring should be performed on the neonates with the risk factors, such as premature rupture of membranes, maternal antepartum fever, complication of GDM, and elevation of serum procalcitonin, to reduce the risk of developing intrauterine infectious pneumonia.
