【摘要】 目的探讨妇科手术后患者并发静脉血栓栓塞症(VTE)的危险因素。方法选择行各类妇科手术患者368例,其中术后并发VTE 33例(血栓组),未并发VTE 335例(非血栓组),收集两组患者的临床资料,分析可能引发VTE的因素。结果 368例患者术后并发VTE 33例,发生率为8.97%。多因素非条件Logistic回归分析结果显示:患者年龄大、术前合并糖尿病、高血压、肺炎、肺结核、心脑血管疾病及术后血清D-二聚体水平高、术中出血量多、手术时间长均为妇科手术后患者并发VTE的独立危险因素。结论患者年龄大、术前合并糖尿病、高血压、肺炎、肺结核及心脑血管疾病及术后血清D-二聚体水平高、术中出血量多、手术时间长是妇科术后患者并发VTE的独立危险因素,术前应采取相应的预防措施,以降低术后VTE的发生率。更多还原

【Abstract】 Objective To investigate the risk factors for concurrent venous thromboembolism(VTE) in patients after gynecological surgery.Methods A total of 368 patients undergoing various procedures of gynecological surgery were enrolled, including 33 cases of postoperative complication of VTE(thrombosis group) and 335 cases without complication of VTE(non-thrombosis group), and the clinical data of patients in the two groups were collected to analyze the potential factors for developing VTE.Results Thirty-three of the 368 patients were complicated with VTE after operation, with an incidence rate of 8.97%. Multivariate non-conditional Logistic regression analysis results showed that older patients, complication of preoperative diabetes, hypertension, pneumonia, tuberculosis and cardiovascular diseases, as well as high serum D-dimer level postoperatively, massive intraoperative hemorrhage and prolonged operation duration were the independent risk factors for concurrent VTE in patients after gynecological surgery. Conclusion Older patients, complication of preoperative diabetes, hypertension, pneumonia, tuberculosis and cardiovascular disease, as well as high serum D-dimer level postoperatively, massive intraoperative hemorrhage and prolonged operation duration are the independent risk factors for concurrent VTE in patients after gynecological surgery. Corresponding preventive measures should be taken before surgery to reduce the incidence rate of postoperative VTE.更多还原
