【摘要】 目的探讨改良Bevan术在小儿高位隐睾伴腹股沟斜疝中的临床应用价值。方法选取2018年8月～2020年3月本院收治的单侧隐睾并腹股沟斜病患儿80例,按照随机数字表法分为两组,每组各40例。观察组实施改良Bevan术式,对照组实施Saha-Jeap术,比较两组手术相关指标变化情况;比较两组麻醉相关指标变化情况;比较两组术后6h机体应激因子水平变化情况;比较术后并发症情况。结果观察组手术时间短于对照组(P <0.05),术后下床时间及术后肛门排气时间均早于对照组(P <0.05),术后住院时间短于对照组(P <0.05),观察组麻醉苏醒时间和麻醉拔管时间均短于对照组(P <0.05),观察组皮质醇和去甲肾上腺均显著低于对照组(P <0.05),观察组发生鞘膜积液、睾丸回缩及术后感染的比例显著低于对照组(P <0.05)。结论针对小儿高位隐睾伴腹股沟斜疝实施改良Bevan术,手术及麻醉恢复快,对患儿应激刺激小,术后并发症少,安全性高。更多还原

【Abstract】 Objective To investigate the clinical application value of modified Bevan operation in children with high cryptorchidism complicated with indirect inguinal hernia.Methods A total of 80 children with unilateral cryptorchidism complicated with indirect inguinal hernia admitted to our hospital from August 2018 to March 2020 were selected and divided into the observation group(n=40) and the control group(n=40) according to the random number table method.The observation group was treated with modified Bevan operation,while the control group was treated with Saha-Jeap operation.The changes of operation-related indexes,anesthesia related indexes,between the two groups were compared.The changes of stress factors levels in the two groups at 6 hours after operation and postoperative incidences of complications were compared.Results The operation duration of the observation group was shorter than that of the control group(P < 0.05).The time of off-bed activity and anus exhausting after operation were both earlier than those in the control group(P < 0.05).The postoperative hospitalization time was shorter than that of the control group(P < 0.05).The time of waking up from anesthesia and anesthesia extubation time in the observation group were both shorter than those in the control group(P < 0.05).Cortisol and noradrenalin in the observation group were both significantly lower than those in the control group(P < 0.05).The proportions of hydrocele,testicular retraction and postoperative infection in the observation group were significantly lower than those in the control group(P < 0.05).Conclusion The modified Bevan operation for children with high cryptorchidism complicated with indirect inguinal hernia has the advantages of quick recovery from operation and anesthesia,less stress stimulation to children,less postoperative complications and higher safety.更多还原
