产前超声指标在早孕期重度α-地中海贫血胎儿中的临床研究
Clinical study on prenatal ultrasound indexes in fetuses with severe α-thalassemia during early pregnancy
【摘要】 目的研究产前超声指标在早孕期重度α-地中海贫血胎儿中的临床价值。方法选取2018年1~12月我院收治的重度α-地中海贫血高危产妇94例为研究对象,同时选取同时期健康孕妇100例为健康组,两组研究对象均接受常规二维超声检查及彩色多普勒超声检查,对大脑中动脉收缩期血流峰、心胸面积、胎盘厚度指标进行比较。结果观察组中重型α-地中海贫血大脑中动脉收缩期血流峰值指标显著高于其他组,差异有统计学意义(P <0.05)。观察组中重型α-地中海贫血心胸面积指标显著高于其他组,差异有统计学意义(P <0.05)。观察组重型α-地中海贫血胎盘厚度指标明显高于同组其他类型及健康组,差异有统计学意义(P <0.05)。结论产前超声指标在早孕期重度α-地中海贫血胎儿中检查具有较高的诊断优势,而对于其他类型的α-地中海贫血胎儿的诊断应结合多想指标进行判断,提高临床诊断率。
【Abstract】 Objective To study the clinical value of prenatal ultrasound indexes in fetuses with severe α-thalassemia during early pregnancy. Methods During the study period from January 2018 to December 2018, 94 high-risk pregnant women with severe α-thalassemia admitted and treated in the department were selected as the study subjects, and 100 healthy pregnant women of the same period were selected as the healthy group. The two groups of study subjects received conventional two-dimensional ultrasonography and color doppler ultrasonography. The indexes of blood flow peak, cardiothoracic area, and placental thickness in systolic period of middle cerebral artery were compared. Results The blood flow peak index in systolic period of middle cerebral artery with severe α-thalassemia in the observation group was significantly higher than that in other groups(P < 0.05). The cardiothoracic area index for severe α-thalassemia in the observation group was significantly higher than that in other groups(P < 0.05). The placental thickness index for severe α-thalassemia in the observation group was significantly higher than other types in the same group and that in the healthy group, with statistically significant difference(P < 0.05). Conclusion Prenatal ultrasound indexes have a higher diagnostic advantage in the examination of fetuses with severe α-thalassemia during early pregnancy, while the diagnosis of fetuses with other types of α-thalassemia shall be judged in combination with multiple indexes to improve the clinical diagnosis rate.
