益生菌制剂对慢性肠炎患儿T淋巴细胞亚群及炎症因子水平的影响

【摘要】 目的:探讨益生菌制剂辅助治疗慢性肠炎患儿的效果,并分析其对T淋巴细胞亚群及炎症因子水平的影响。方法:将2017年10月至2019年9月徐州医科大学附属徐州儿童医院收治的慢性肠炎患儿104例随机分为联合组和常规组,每组52例。常规组给予头孢唑肟钠~+蒙脱石散治疗,联合组在常规组治疗的基础上联合使用益生菌制剂(双歧杆菌四联活菌片)辅助治疗,均治疗3周。对比临床疗效、治疗前后T淋巴细胞亚群水平(CD3~+、CD4~+、CD8~+和CD4~+/CD8~+)、治疗前、后血清炎症因子水平[高敏C反应蛋白(hs-CRP)、干扰素-γ(IFN-γ)、血清白细胞介素-2(IL-2)及血清白细胞介素-10(IL-10)]及总不良事件发生率。结果:联合组总有效率显著高于常规组(P<0.05)。治疗后,两组患者CD8~+与hs-CRP、IL-10水平均较治疗前下降(P<0.05),且联合组均低于常规组(P<0.05);两组患者CD3~+、CD4~+、CD4~+/CD8~+与IFN-γ、IL-2水平均较治疗前上升(P<0.05),且联合组均高于常规组(P<0.05)。两组总不良事件发生率比较,差异无统计学意义(P>0.05)。结论:益生菌制剂可提高慢性肠炎患儿的临床疗效,平衡T淋巴细胞亚群水平,降低炎症水平,且不显著增加总不良事件发生率。

【Abstract】 Objective: To explore the effect of probiotics on T lymphocyte subsets and inflammatory factors levels in children with chronic enteritis. Methods: A total of 104 children with chronic enteritis treated in our hospital from October 2017 to September 2019 were randomly divided into a combined group and a conventional group, with 52 cases in each group. The children in conventional group were treated with ceftizoxime sodium and smectite powder, and those in the combined group were treated as the conventional group with addition of probiotics(Bifidobacterium quadruple viable tablet) for 3 weeks. The clinical efficacy, T lymphocyte subsets(CD3~+,CD4~+, CD8~+and CD4~+/CD8~+) before and after treatment, the levels of serum inflammatory factors(hs-CRP, IFN-γ, IL-2 and IL-10) and the total adverse events were compared. Results: The total efficiency of the combined group was significantly higher than that of the conventional group(P<0.05). After treatment, the levels of CD8~+,hs-CRP and IL-10 in both groups were lower than those before treatment(P<0.05), and the changes were more significant in the combined group(P<0.05). The incidence of total adverse events were similar in the two groups(P>0.05). Conclusion: Probiotics could improve the clinical efficacy of children with chronic enteritis, balance the level of T lymphocyte subsets, reduce inflammatory response, and did not increase the total incidence of adverse events.
