维生素D和MBCI在评估2型糖尿病患者β细胞功能中的应用
Application of vitamin D and MBCI in evaluating β-cell function in patients with type 2 diabetes
【摘要】 目的研究新β细胞功能指数(MBCI)和维生素D联合评估2型糖尿病患者β细胞功能的可能性。方法回顾分析22例糖耐量正常者、31例2型糖尿病患者的口服葡萄糖耐量试验(OGTT)、胰岛素、维生素D、糖化血红蛋白,计算Homa-β胰岛素分泌指数(HBCI)、MBCI、ΔI60/ΔG60、Homa-胰岛素抵抗指数(HomaIR)。利用参数检验,线性回归分析维生素D与不同β细胞功能指数组合和血糖水平变化的相关性。结果维生素D与糖化血红蛋白不相关(r=-0.349,P>0.05),维生素D与OGTT试验60min血糖(PG60)、PG120显著相关(r=-0.373、-0.408,均P<0.05)。以PG120为因变量,分别以维生素D+HBCI、维生素D+MBCI、维生素D+ΔI60/ΔG60的组合为自变量做线性回归分析,所得模型相关指数R2分别为0.290、0.174、0.293;以HomaIR+HBCI、Homa-IR+MBCI、Homa-IR+ΔI60/ΔG60的组合为自变量做线性回归分析,所得模型相关指数R2分别为0.576、0.789、0.493;以Homa-IR+HBCI+维生素D、Homa-IR+MBCI+维生素D、Homa-IR+ΔI60/ΔG60+维生素D的组合为自变量做线性回归分析,所得模型相关指数R2分别为0.607、0.783、0.569。结论维生素D与PG60、PG120显著相关,但与MBCI的组合仅能解释一小部分血糖水平的变化。在评估2型糖尿病患者β细胞功能时,MBCI较其他β细胞功能指数更具可靠性,能解释更多的血糖变化。
【Abstract】 Objective To study the possibility of the modifiedβ-cell function index(MBCI)and vitamin D jointly evaluating β-cell function in patients with type 2 diabetes.Methods The oral glucose tolerance test(OGTT),insulin,vitamin D,and glycated hemoglobin were retrospectively analyzed in 22 patients with normal glucose tolerance and 31 patients with type 2 diabetes,Homa-β insulin secretion index(HBCI),MBCI,ΔI60/ΔG60,Homa-insulin resistance index(Homa-IR)were calculated.Parametric tests and linear regression were used to analyze the correlation between vitamin D and different β-cell function index combinations with changes in blood glucose levels.Results Vitamin D was not correlated with glycated hemoglobin(r=-0.349,P>0.05),and it was significantly correlated with OGTT test 60 min blood glucose(PG60)and PG120(r=-0.373,-0.408,both P<0.05).Linear regression analysis was performed using PG120 as the dependent variable,and vitamin D+HBCI,vitamin D+MBCI,and vitamin D+ΔI60/ΔG60 as independent variables,the model correlation index R2 were 0.290,0.174,0.293,respectively;linear regression analysis was performed using Homa-IR+HBCI,Homa-IR+MBCI,Homa-IR+ΔI60/ΔG60 as independent variables,and the correlation index R2 of the model were 0.576,0.789,0.493,respectively;linear regression analysis was performed using Homa-IR+HBCI+vitamin D,Homa-IR+MBCI+vitamin D,Homa-IR+ΔI60/ΔG60+vitamin D as independent variables,and the correlation index R2 of the model were 0.607,0.783,0.569.Conclusion Vitamin D is significantly related to PG60 and PG120,but the combination with MBCI can only explain a small part of the changes in blood glucose levels.In assessingβ-cell function in patients with type 2 diabetes,MBCI is more reliable than other β-cell function indexes and can explain more blood glucose changes.
