【摘要】 目的比较经尿道前列腺汽化电切术(TUVP)与经尿道前列腺钬激光剜除术(HoLEP治疗老年良性前列腺增生(BPH)的临床效果。方法选取2014年1月～2016年12月吉林大学第二医院泌尿外科收治的490例BPH患者,根据术式分为TUVP组与HoLEP组,比较两组手术时间、出血量、术中和术后并发症;术前与术后的国际前列腺症状评分(IPSS)、生活质量(QOL)评分、最大尿流率(Qmax)和剩余尿量(PVR)。结果 HoLEP组术中出血量及术后并发症发生率明显少于TUVP组(P <0.05)。术后12个月与术前相比,两组IPSS明显降低,Qmax及PVR明显改善,QOL评分明显降低(P <0.05);但组间比较,IPSS、Qmax、PVR及QOL评分差异无统计学意义(P> 0.05)。结论 TUVP与HoLEP治疗BPH疗效确切,HoLEP术中出血量更少、术后并发症发生率小,值得更好地临床推广。更多还原

【Abstract】 Objective To compare the clinical efficacy of transurethral vaporization of prostate(TUVP) and transurethral holmium laser enucleation(Ho LEP) in the treatment of benign prostatic hyperplasia(BPH).Methods 490 patients with BPH in the Urology Department of the Second Hospital of Jilin University from January 2014 to December 2016 were divided into TUVP group and Ho LEP group according to the surgical methods.The operation time,bleeding volume,intraoperative and postoperative complications,International Prostate Symptom Score(IPSS),quality of life(QOL),maximum flow rate(Qmax) and residual urine volume(PVR) were compared between the two groups.Results The amount of intraoperative bleeding and the incidence of postoperative complications in the Ho LEP group were significantly lower than those in the TUVP group(P < 0.05).12 months after operation,the IPSS was lower,Qmax was higher,PVR and QOL scores were lower than those before operation in the two groups(P < 0.05).However,there was no significant difference in IPSS,Qmax,PVR and QOL scores between the two groups(P > 0.05).Conclusion TUVP and Ho LEP are effective in the treatment of BPH.Ho LEP has less intraoperative bleeding and less postoperative complications.更多还原
