柠檬冰刺激结合低频电刺激治疗脑卒中后吞咽功能障碍疗效观察

【摘要】 目的:观察柠檬冰刺激结合低频电刺激治疗脑卒中后吞咽功能障碍的治疗效果以及安全性。方法:将脑卒中后吞咽障碍患者120例根据随机数字表原则分为4组,每组30例。对照组患者采用常规治疗,治疗1组在对照组基础上联合柠檬冰刺激,治疗2组在对照组基础上联合低频电刺激,治疗3组患者在对照组基础上联合柠檬冰刺激和低频电刺激。比较4组患者治疗前后功能性经口摄食量表(functional oral intake scale,FOIS)评级、视频透视吞咽检查(video fluoroscopic swallowing study,VFSS)评分、洼田饮水试验(water swallow test,WST)评级、吞咽肌障碍评分及不良反应。结果:治疗后4组患者FOIS及WST评级均优于治疗前,差异具有统计学意义(P<0.05)。治疗1组、2组及3组患者FOIS评级、WST评级优于对照组,差异具有统计学意义(P<0.05)。治疗3组患者FOIS评级、WST评级优于治疗1组、2组,差异具有统计学意义(P<0.05)。治疗1组与治疗2组患者FOIS评级、WST评级比较,差异无统计学意义(P>0.05)。治疗后4组患者VFSS评分、吞咽肌障碍评分均高于治疗前,差异具有统计学意义(P<0.05)。治疗1组、2组及3组患者VFSS评分、吞咽肌障碍评分高于对照组,差异具有统计学意义(P<0.05)。治疗3组患者VFSS评分、吞咽肌障碍评分高于治疗1组、2组,差异具有统计学意义(P<0.05)。治疗1组与治疗2组患者VFSS评分、吞咽肌障碍评分比较差异无统计学意义(P>0.05)。4组间不良反应发生率比较差异无统计学意义(P>0.05)。结论:柠檬冰刺激和低频电刺激均可有效改善脑卒中后吞咽功能障碍患者吞咽功能,但二者联合应用疗效更优,并且安全性与两者单用相当。

【Abstract】 Objective: To analyze the therapeutic effect and safety of lemon ice stimulation combined with low frequency electrical stimulation in the treatment of swallowing dysfunction(SD) after stroke. Methods: A total of 120 patients with SD after stroke were divided into four groups according to the principle of random number table, 30 cases each group. The control group used conventional therapy, the treatment group No. 1 accepted lemon ice stimulation based on the treatment the control group accepted, the treatment group No.2 low frequency electrical stimulation on the foundation of the treatment the control group accepted, and the treatment group No.3 both stimulation therapies. To compare FOIS grade, VFSS scores, WST rate, dysphagia score and adverse reactions between four groups before and after treating. Results: After treating, FQIS and WST grades of four groups were superior to these before treating, and the difference was statistically significant(P<0.05). FQIS and WST grades of the treatment groups of NO.1, 2 and 3 were better than these of the control group, and the difference had statistical meaning(P<0.05). FQIS and WST grades of the treatment groups NO. 3 were better than these of the treatment groups of NO. 1 and 2, and the difference had statistical meaning(P <0.05). The difference had no statistical meaning in FQIS and WST grades between the treatment groups of No.1 and 2(P >0.05). After treating, VFSS scores and dysphagia scores of the four groups were higehr than these before treating, and the difference was statistically significant(P<0.05). VFSS scores and dysphagia scores of the treatment group NO. 3 were higher than these of the treatment groups of NO. 1 and 2, and the difference was statistically significant(P<0.05). The difference showed no statistical meaning in VFSS and dysphagia scores between the treatment groups of NO. 1 and 2(P>0.05).The difference showed no statistical meaning in the incidence of adverse reaction between four groups(P>0.05).Conclusion: Lemon ice stimulation combined with low frequency electrical stimulation could effectively improve swallowing function of the patients, their combination possess better clinical effects and they present similar safety.
