口腔颌面部间隙感染的临床特征分析与治疗方法
Clinical characteristics and treatment of oral and maxillofacial space infection
【摘要】 目的探讨口腔颌面部间隙感染的临床特征与治疗方法。方法选取本院2015年3月至2019年3月收治的300例口腔颌面部间隙感染患者,按照随机数字法分为两组,各150例。对照组选择常规治疗,实验组在常规治疗的基础上应用高压氧联合脓肿切开引流术治疗,分析病原菌种类,并比较两组疗效。结果 300例患者中,182例患者为蜂窝组织炎症,占比60.67%(182/300);118例患者为脓肿,占比39.33%(118/300)。感染部位包括嚼肌间隙(14.67%)、颊间隙(6.67%)、眶下间隙(23.33%)、颌下间隙(37.67%)等,感染源包括外伤性感染(13.67%)、腺源性感染(26.67%)、牙源性感染(58.33%)等。300例患者共培养出病原菌232株,包括98株革兰阳性菌(58株金黄色葡萄球菌、40株溶血性链球菌)、134株革兰阴性菌(37株大肠埃希菌、61株铜绿假单胞菌、17株肺炎克雷伯菌、19株阴沟肠杆菌)。实验组治疗总有效率为93.33%,明显高于对照组的81.33%(P<0.05)。结论口腔颌面部间隙感染主要发生于颌下间隙,感染源主要为牙源性感染,主要是因革兰阴性菌所致;在常规治疗的同时,应用高压氧联合脓肿切开引流术治疗可明显提高临床疗效,具有临床推广价值。
【Abstract】 Objective To investigate the clinical characteristics and treatment of oral and maxillofacial space infection.Methods 300 patients with oral and maxillofacial space infection in our hospital from March 2015 to March 2019 were selected and randomly divided into two groups,with 150 cases in each group.The control group was given conventional treatment,while the experimental group was treated with hyperbaric oxygen combined with abscess incision and drainage on the basis of conventional treatment.The types of pathogenic bacteria were analyzed and the curative effect of the two groups was compared.Results In 300 patients,182 patients were honeycomb tissue inflammation,accounting for 60.67%(182/300).118 patients were abscess,accounting for 39.33%(118/300).The infection sites included masseter space(14.67%),buccal space(6.67%),infraorbital space(23.33%) and submandibular space(37.67%).The sources of infection included traumatic infection(13.67%),adenogenic infection(26.67%),odontogenic infection(58.33%),etc.A total of 232 strains of pathogenic bacteria were cultured from 300 patients,including 98 strains of Gram-positive bacteria(58 strains of Staphylococcus aureus and 40 strains of hemolytic streptococcus),134 strains of Gram-negative bacteria(37 strains of Escherichia coli,61 strains of Pseudomonas aeruginosa,17 strains of Klebsiella pneumoniae and 19 strains of Enterobacter cloacae).The total effective rate of the experimental group was 93.33%,which was significantly higher than 81.33% of the control group(P<0.05).Conclusion Oral and maxillofacial space infection mainly occurs in the submaxillary space,the main source of infection is odontogenic infection,mainly caused by gram-negative bacteria.In the conventional treatment at the same time,the application of hyperbaric oxygen combined with abscess incision and drainage treatment can significantly improve the clinical efficacy,and has clinical promotion value.
