【摘要】 目的探讨经皮自体骨髓血注射结合局部微骨折技术治疗四肢骨不连。方法选取2016年1月至2018年1月采用经皮自体骨髓血注射结合局部微骨折技术治疗四肢骨不连患者15例。使用16号骨穿针在髂前上棘后方髂棘处抽取骨髓血,C臂透视下确认骨折断端位置后,使用2.0克氏针经皮在骨折断端钻孔人为造成局部多处微骨折,透视下注射入骨折断端微骨折部位。结果 15例患者全部获得10个月以上的随访,随访时间8～20个月,平均(12.3±5.7)个月。评价标准使用Murley标准评价临床疗效,末次随访时,优11例,良2例,可1例,另1例内固定断裂,再次手术更换内固定,优良率86.7%(13/15)。结论本组所采用的经皮自体骨髓血注射结合局部微骨折技术治疗四肢骨不连,具有创伤小、费用少,疗效可靠等优点,是治疗四肢长骨骨折术后骨不连的一种新选择。更多还原

【Abstract】 Objective To explore the treatment of nonunion of limbs by percutaneous autologous bone marrow blood injection combined with local micro-fracture technique. Methods From January 2016 to January 2018, 15 patients with nonunion of limbs were treated by percutaneous autologous bone marrow blood injection combined with local micro-fracture technique. Bone marrow blood was extracted from the iliac spine behind the anterior superior iliac spine with the No. 16 bone penetrating needle. After the fracture site was confirmed by C-arm fluoroscopy, a 2.0 K-wire was used to percutaneously drill through the fracture site to cause multiple local micro-fractures. Under fluoroscopy, the bone marrow blood was injected into the fracture site. Results All the 15 patients were followed up for more than 10 months. The follow-up time ranged from 8 to 20 months, with an average of 12.8 months. Murley’ s criteria were used to evaluate the clinical efficacy. At the last follow-up, 11 cases were excellent, 2 cases were good, 1 case was fair, and the other one was broken. The excellent and good rate was 86.7%(13/15) when internal fixation was replaced by reoperation. Conclusion The technique of percutaneous autologous bone marrow blood injection combined with local micro-fracture for the treatment of nonunion of limbs has the advantages of less trauma, less cost and reliable curative effect. It is a new choice for the treatment of nonunion of long bone fracture of limbs after operation.更多还原
