ICU脑膜脓毒性伊丽莎白金菌医院感染暴发调查

【摘要】 目的:探讨重症监护病房(ICU)发生的脑膜脓毒性伊丽莎白金菌(EM菌)医院感染的原因。方法:调查ICU 3例脑膜脓毒性伊丽莎白金菌检出病例,对分离菌株进行药敏试验和同源性分析,对环境标本进行目标微生物监测。结果:3例重度肺炎患者经同一重复消毒的纤维支气管镜治疗后,肺泡灌洗液均检出了脑膜脓毒性伊丽莎白金菌,相继检出间隔时间不超过3 d,同期环境卫生学监测结果显示在ICU呼吸机和治疗台面检出脑膜脓毒性伊丽莎白金菌。结论:纤维支气管镜是ICU患者EM菌疑似感染暴发的潜在传染源。医务人员接触不清洁环境物表可能是此次EM菌医院感染暴发的重要传播媒介,做好环境清洁消毒和手卫生可防止EM进一步蔓延。

【Abstract】 Objective: Three infected cases with Elizabethkingia meningoseptica were investigated in the intensive care unit, with the aim of preventing the outbreak of nosocomial infection and finding the possible source and route of infection.Methods:We have analyzed dinical features of the infected cases with Elizabethkingia meningoseptica in ICU,carried out drug sensitivity test on the isolated strains, and monitored the target microorganism on the environmental samples. Results: Three patients with severe pneumonia were treated with the same fiberbronchoscope, and Elizabethkingia meningoseptica was detected in the alveolar lavage fluid. The time interval between the detection was no more than 3 days. At the same time, environmental hygiene monitoring resutts showed that Elizabethkingia meningoseptica was detected in the ventilator and treatment table of ICU. Conclusion: Fiberbronchoscope may be the potential source of EM infection in ICU. Inadequate environmental cleanness maybe the bacterial route of transmission.Disinfection of environmental substances and cleanness of medical personnel hands can prevent EM from further spreading.
