初探不同标本核酸检测在新型冠状病毒肺炎患者诊断中的应用
Preliminary Study on the Application of Nucleic Acid Detection of Different Specimens on the Diagnosis of COVID-19 Patients
【摘要】 目的:了解新型冠状病毒肺炎(COVID-19)患者不同类型标本进行核酸检测阳性结果的差异,以及多种类型的标本联合检测后对结果准确性的判断,为COVID-19的诊断及治疗效果观察提供依据。方法:对COVID-19患者在诊断及治疗过程中的咽拭子、痰液、粪便、血清及血浆标本的核酸检测结果进行统计,同时对COVID-19患者在不同治疗阶段多种类型的标本联合检测的结果进行综合判断。结果:COVID-19患者咽拭子、痰液标本的核酸检测结果存在一定差异,其中痰液标本的阳性结果占比高于咽拭子,而血清和血浆标本均未出现阳性结果,对多种类型的标本联合检测可提高核酸检测结果判断的准确性。结论:核酸检测存在假阴性情况,在进行结果判断时不能孤立地进行,应通过多次多种类型的标本联合检测综合判断检测结果,并以此作为临床诊断的依据。
【Abstract】 Objective: To understand the difference in the results of nucleic acid test of different types of specimens of patients with novel coronavirus pneumonia(COVID-19) and judge the accuracy of the results after the combined detection of various types of specimens, so as to provide a basis for the diagnosis and treatment of COVID-19. Methods: The nucleic acid test results of throat swabs, sputum, feces and serum or plasma samples in the course of diagnosis and treatment of patients with COVID-19 were analyzed retrospectively and the combined detection results of various types of specimens of COVID-19 patients at different treatment stages were comprehensively judged. Results: There were some differences in the nucleic acid detection results of pharyngeal swabs and sputum specimens in COVID-19 patients. The positive results of sputum specimens were higher than that of pharyngeal swabs, but neither serum nor plasma specimens showed positive results. The combined testing of specimens could greatly improve the accuracy of nucleic acid detection. Conclusion: Nucleic acid testing has a large degree of false negatives, which cannot be conducted in isolation when judging the results. Multiple tests of multiple types of specimens should be used to comprehensively judge the test results and this should be used as the basis for clinical diagnosis.
