呼吸机模拟高压氧治疗对重度颅脑创伤患者急性期凝血功能影响研究
Effect of ventilator simulated hyperbaric oxygen therapy on coagulation function in acute stage of severe craniocerebral trauma
【摘要】 目的探讨呼吸机模拟高压氧治疗对重型颅脑创伤患者急性期凝血功能的影响。方法选取自2017年1月至2018年6月广西壮族自治区人民医院收治的75例重度颅脑创伤患者为研究对象。根据随机数字表法将患者分为常规治疗组(n=39)和模拟高压氧治疗组(n=36)。检测两组患者的凝血功能,分析呼吸机模拟高压氧治疗对患者急性期凝血功能的影响。结果治疗前,两组患者凝血功能比较,差异无统计学差异(P>0.05);治疗后,模拟高压氧治疗组患者部分凝血活酶时间、凝血酶原时间、凝血酶时间、D-二聚体、血浆纤维蛋白原降解产物、纤维蛋白原、血浆凝血酶原片段F1+2及凝血酶调节蛋白均明显低于常规治疗组,差异均有统计学意义(P<0.05)。治疗后,两组患者均无恢复良好患者,模拟高压氧治疗组患者ICU住院时间明显短于常规治疗组,中度残疾患者所占比例明显高于常规治疗组,两组间比较,差异有统计学意义(P<0.05)。结论呼吸机模拟高压氧治疗有利于颅脑创伤患者急性期凝血、纤溶系统的恢复,可减少ICU住院时间。
【Abstract】 Objective To investigate the effect of ventilator hyperbaric oxygen therapy on acute coagulation in patients with severe craniocerebral trauma.Methods A retrospective study was performed on 75 cases of patients with severe craniocerebral trauma who were admitted from January 2017 to June 2018.Patients were randomly divided into the conventional group(n=39)and the hyperbaric oxygen treatment group(n=36).The coagulation function of the two groups of patients was detected,and the effect of ventilator hyperbaric oxygen therapy on the coagulation function of the patients in the acute stage was analyzed.Results Before treatment,there was no statistically significant difference in coagulation function between the two groups(P>0.05).Prothrombin time,prothrombin time,prothrombin time,D-Dimer,plasma fibrinogen degradation products,fibrinogen,plasma prothrombin fragment F1+2 and prothrombin regulatory protein were significantly lower than those in the conventional treatment group(P<0.05).After treatment,no patients in the two groups had a good recovery.The ICU stay of patients in the simulated hyperbaric oxygen treatment group was significantly shorter than that in the conventional treatment group,and the number of patients with moderate disability was significantly higher than that in the conventional treatment group(P<0.05).Conclusion Ventilator simulated hyperbaric oxygen treatment is beneficial to the recovery of coagulation and fibrinolysis systems in patients with acute craniocerebral trauma,and can reduce the length of stay in ICU.
