甘露糖结合凝集素基因多态性及血清表达与慢性阻塞性肺疾病患者急性加重的相关性研究

【摘要】 目的:探讨甘露糖结合凝集素(mannose-binding lectin,MBL)基因多态性及其血清表达与COPD急性加重的相关性。方法:采用病例-对照研究,COPD组63例、对照组54例,COPD组根据急性加重次数进行分组,利用聚合酶链反应(PCR)和限制性内切酶片断长度多态性技术(RFLP)测定MBL2基因第54位密码子的多态性,利用酶联免疫吸附法(ELISA)测定血清MBL水平。结果:COPD组与对照组MBL2基因第54位密码子基因型频率和等位基因频率差异无统计学意义(P>0.05);COPD组MBL水平低于对照组,COPD组中AB、BB基因型血清MBL水平低于AA基因型(均为P<0.05);COPD反复加重组与非反复加重组基因型分布、等位基因频率差异有统计学意义(P<0.05),B等位基因在COPD反复加重组的频数高于COPD非反复加重组。结论:在内蒙古包头汉族人群中,COPD患者与正常人群的MBL2基因第54位密码子基因型差异无统计学意义,但低MBL水平可能与COPD发病有关;COPD患者MBL2第54位密码子多态性可影响血清MBL表达水平,且与COPD患者急性加重次数相关,B等位基因可能是COPD急性加重的易感基因。

【Abstract】 Objective:To observe the correlation between mannose-binding Lectin(MBL) gene polymorphism and its serum expression and acute exacerbation of COPD. Methods: Case-control study was conducted, with 63 cases in the COPD group and 54 cases in the control group. The COPD group was grouped according to the frequency of acute exacerbations. The polymorphism of the 54 th codon of MBL2 gene was determined by polymerase chain reaction(PCR) and restriction enzyme fragment length polymorphism(RFLP). Serum MBL level was determined by enzyme-linked immunosorbent assay(ELISA). Results: There were no statistically significant differences in the genotype frequency and allele frequency of the 54 th codon of MBL2 gene between the COPD group and the control group(P>0.05). The MBL level of COPD group was lower than that of the control group, and the serum MBL levels of AB and BB genotypes in COPD group were lower than those of AA genotype, there being statistically significant differences(P<0.05).The genotype distribution and allele frequency of repeated plus recombination and non-repeated plus recombination in COPD showed statistically significant differences(P<0.05); the frequency of repeated plus recombination of B allele in COPD was significantly higher than that of non-repeated plus recombination in COPD. Conclusion: The MBL2-54 gene polymorphism may be unrelated to COPD susceptibility in the Baotou Han population in Inner Mongolia, but low MBL levels may be related to the incidence of COPD. MBL2-54 polymorphism in COPD patients can affect the expression levels of serum MBL, and is related to the frequency of acute exacerbations in COPD patients, and B allele may be the susceptibility gene for acute exacerbations of COPD.
