低钙血症与慢性心力衰竭的相关性研究
Correlation between Hypocalcemia and Chronic Heart Failure
【摘要】 目的:探讨慢性心力衰竭(CHF)与低钙血症的相关性。方法:搜集2017年2月~2019年3月因CHF入住我院的患者,分析比较不同心功能分级患者血清Ca2+水平、低钙血症发生率及血清Ca2+水平与血肌酐、血尿酸、N末端B型利钠肽前体(NT-pro BNP)、左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)的相关性。结果:心功能Ⅱ级组血清Ca2+水平(2.14±0.17mmol/L)明显高于Ⅲ级组(2.08±0.15mmol/L)及Ⅳ级组(2.03±0.14mmol/L),差异均有统计学意义(均P<0.01);心功能Ⅱ级组(58.82%)低钙血症发生率明显低于Ⅲ级组(79.75%)和Ⅳ级组(86.96%),差异均有统计学意义(均P<0.01)。与正常血钙组相比,低钙血症组血肌酐、血尿酸、NT-pro BNP和LVEDD升高,而LVEF降低,差异均有统计学意义(均P<0.01)。血清Ca2+水平与血肌酐、血尿酸、NT-pro BNP、LVEDD呈负相关,与LVEF呈正相关(均P<0.01)。结论:CHF患者血清Ca2+水平与心功能级别相关,可作为患者预后的预测因子。
【Abstract】 Objective:To investigate the correlation between chronic heart failure(CHF)and hypocalcemia.Methods:Patients admitted to our hospital due to CHF from February 2017 to March 2019 were collected.The serum Ca2+level and the incidence of hypocalcemia in patients with different grades of cardiac function were analyzed and compared.The correlation between serum Ca2+level and serum creatinine,blood uric acid,N-terminal pro-B-type natriuretic peptide(NT-pro BNP),left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD)were analyzed.Results:The serum Ca2+levels of patients with cardiac function grade II(2.14±0.17 mmol/L)was significantly higher than that of grade Ⅲ(2.08±0.15 mmol/L)and grade IV(2.03±0.14 mmol/L)(both P <0.01).The incidence of hypocalcemia in cardiac function grade II(58.82%)was significantly lower than that of grade Ⅲ(79.75%)and grade IV(86.96%),with statistically significant difference(both P<0.01).The serum creatinine,serum uric acid,NT-pro BNP and LVEDD were increased in the hypocalcemia group compared with the normal group,while the LVEF was decreased(all P<0.01).The serum Ca2+level was negatively correlated with serum creatinine,serum uric acid,NT-pro BNP and LVEDD,while positively correlated with LVEF(all P<0.01).Conclusion:The level of serum Ca2+in CHF patients is related to the grade of cardiac function,which can be used as a predictor for CHF prognosis.
