【摘要】 目的探讨醋酸兰瑞肽辅助伽玛刀治疗生长激素(GH)型垂体腺瘤的临床疗效及对GH和胰岛素样生长因子1(IGF1)水平的影响。方法依据治疗方案的不同将65例GH型垂体腺瘤患者分为对照组(n=32)和观察组(n=33),对照组患者采用伽玛刀进行治疗,观察组患者在对照组的基础上联合使用醋酸兰瑞肽辅助治疗。比较两组患者治疗后的临床疗效和内分泌学疗效,比较治疗前及治疗后两组患者的血清GH和IGF1水平,并记录两组患者的不良反应发生情况。结果治疗后,观察组患者的临床疗效和内分泌学疗效均明显优于对照组,差异均有统计学意义(P﹤0.01)。治疗后,两组患者的血清GH和IGF1水平均低于本组治疗前,且观察组患者的血清GH和IGF1水平均低于对照组,差异均有统计学意义(P﹤0.05)。两组患者的不良反应发生率比较,差异无统计学意义(P﹥0.05)。结论与单独应用伽玛刀治疗相比,醋酸兰瑞肽辅助伽玛刀治疗GH型垂体腺瘤的临床疗效较好,且可降低患者的血清GH和IGF1水平。更多还原

【Abstract】 Objective To investigate the clinical efficacy of gamma knife and adjuvant lanreotide acetate in the treatment of growth hormone(GH)-secreting pituitary adenoma and the effect on serum GH and insulin-like growth factor 1(IGF 1) levels.Method The clinical data of 65 patients with GH-secreting pituitary adenoma were retrospectively analyzed.These patients were divided into control group(n=32) and study group(n=33),with the control group being treated with gamma knife,and study group with gamma knife combined with adjuvant lanreotide acetate.The clinical efficacy and endocrine efficacy were evaluated in all patients,the serum GH and IGF1 levels before and after treatment were compared between the two groups,additionally,the occurrence of adverse reactions in both groups were recorded.Result The clinical and endocrine efficacy in study group were significantly better than that in control group after treatment(P<0.01).Decreased levels of serum GH and IGF1 in both groups were evident after treatment,with significantly lower levels of serum GH and IGF1 in study group versus control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment of GH-secreting pituitary adenoma patients with gamma knife and adjuvant lanreotide acetate demonstrates evident clinical efficacy,with significantly reduced serum GH and IGF1 levels.更多还原
