【摘要】 目的:研究分析丁苯酞联合阿司匹林、氯吡格雷片对行神经介入手术治疗老年脑血管疾病患者的抗血栓作用和用药安全性。方法:选取2013年2月—2017年7月在我院进行神经介入手术治疗的32例脑血管疾病患者作为研究对象,按治疗方法的不同分为对照组[12例,阿司匹林(300 mg,qd)+氯吡格雷片(75 mg,qd)]和试验组[20例,在对照组的基础上加服丁苯酞(0.2 g,qd)]。所有患者均从术前一周开始服药持续到术后6个月。比较两组治疗效果、血小板聚集率、血流动力学指标及不良反应。结果:治疗后,试验组总有效率高于对照组,差异有统计学意义(P <0.05);治疗后试验组血小板聚集率(PAR)、全血黏度切变率(SR-WBV)、相对血液黏度切变率(SR-RBV)、血浆黏度(PV)、血细胞比容(HCT)及纤维蛋白原(FIB)均低于治疗前,且PAR、SR-WBV、SR-RBV、HCT均低于对照组,差异有统计学意义(P <0.05);两组不良反应发生率比较差异无统计学意义(P> 0.05)。结论:丁苯酞联合阿司匹林、氯吡格雷片对进行神经介入治疗后的老年脑血管疾病患者的抗血栓作用明显,且用药安全,值得临床推广运用。更多还原

【Abstract】 Objective: To study and analyze the antithrombotic effect and use safety of butylphthalide combined with aspirin and clopidogrel tablets in the treatment of cerebrovascular diseases in elderly patients undergoing neurological interventional surgery. Methods: 32 patients with cerebrovascular diseases who underwent neurological interventional surgery in our hospital from February 2013 to July 2017 were selected as subjects investigated, and they were randomly divided into a control group [12 cases, aspirin(300 mg), qd) + clopidogrel tablets(75 mg, qd)] and an experimental group [20 cases, butylphthalide(0.2 g, qd) on the basis of the control group]. All patients took the medicine from one week before surgery to 6 months after surgery. The efficacy, platelet aggregation rate, hemodynamic indexes and adverse reactions were compared between the two groups. Results:After treatment, the total effective rate in the experimental group was higher than that in the control group, and the difference was statistically significant(P<0.05). The platelet aggregation rate(PAR), shear rate of whole blood viscosity(SR-WBV), shear rate of relative blood viscosity(SR-RBV), plasma viscosity(PV), hematocrit(HCT) and fibrinogen(FIB) in the experimental group after treatment were lower than those before treatment, and the PAR, SR-WBV, SR-RBV and HCT in the experimental group were low than those in the control group, with statistically significant differences(P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion: Butylphthalide combined with aspirin and clopidogrel tablets has an obvious antithrombotic effect on elderly patients with cerebrovascular diseases after neurological interventional treatment, and is safe in drug use and worthy of clinical populariz ation and application.更多还原
