不同剂量放疗联合TP化疗治疗中老年食管癌的有效性与安全性
Effectiveness and Safety of Different Doses of Radiotherapy Combined with TP Chemotherapy in the Treatment of Esophageal Cancer in Middle-aged and Elderly Patients
【摘要】 目的:分析不同剂量放疗联合紫杉醇与顺铂联合化疗方案(TP)治疗中老年食管癌的有效性与安全性。方法:选择我院2013年1月—2015年5月收治的中老年食管癌患者100例,根据随机数字表法分为对照组与治疗组,各50例。全部患者均接受TP化疗,并在此基础上联合实施放疗,对照组为低剂量(50.4 Gy)放疗,治疗组为高剂量(60 Gy)放疗,两组均持续接受放疗同步化疗4个周期,比较两组放射性肺损伤发生率及近期疗效。治疗结束后随访2年,记录并比较两组随访结束时的治疗有效率、生存率及局部控制率。结果:治疗组近期控制率高于对照组,差异有统计学意义(P <0.05);两组放射性肺损伤发生率比较,差异无统计学意义(P> 0.05);随访结束时治疗组总控制率、总生存率、局部控制率均高于对照组,差异有统计学意义(P <0.05)。结论:高剂量放疗同步TP方案治疗中老年食管癌,可在不增加放射性肺损伤的情况下,提高患者近期疗效与生存率,安全可靠,值得临床借鉴。
【Abstract】 Objective:To analyze the effectiveness and safety of different doses of radiotherapy combined with paclitaxel and cisplatin combined chemotherapy(TP) in the treatment of esophageal cancer in middle-aged and elderly patients. Methods:100 middle-aged and elderly patients with esophageal cancer admitted to our hospital from January 2013 to May 2015 were selected and randomly divided into a control group and a treatment group, 50 cases each. All patients received TP chemotherapy, and on this basis, radiotherapy was also given. The patients in the control group received low-dose(50.4 Gy) radiotherapy, and the patients in the treatment group received highdose(60 Gy) radiotherapy. The patients in the two groups continuously received concurrent radiotherapy for 4 cycles.The incidence of radiation-induced lung injury and short-term efficacy were compared between the two groups. All patients were followed up for 2 years after treatment. The treatment efficiency, survival rate and local control rate at the end of follow-up were recorded and compared between the two groups. Results:The short-term control rate in the treatment group was higher than that in the control group, and the difference was statistically signi?cant(P<0.05).There was no statistically signi?cant difference in the incidence of radiation-induced lung injury between the two groups(P>0.05). The total control rate, overall survival rate and local control rate at the end of follow-up in the treatment group were higher than those in the control group, and the differences were statistically signi?cant(P<0.05).Conclusion:High-dose radiotherapy combined with TP regimen for the treatment of esophageal cancer in middleaged and elderly patients can improve the short-term efficacy and survival rate without increasing radiation-induced lung injury, which is safe and reliable and worthy of clinical reference.
