【摘要】 目的:探讨高血压特殊合并症患者降压方案的指南依从性,为规范降压治疗、加强血压管理提供参考。方法:抽取某院2017年1～12月合并特殊合并症的高血压患者病历248例,分析降压治疗方案,参照《高血压合理用药指南(第2版)》进行降压方案指南依从性评价,并考察依从性对血压达标率的影响。结果:降压治疗方案的指南依从率为71. 77%,不依从问题包括药物启动时机不依从、方案制定不依从、药物选择不依从、剂量用法不依从等。出院血压达标率为68. 14%,治疗方案依从指南组的达标率是不依从组的2. 34倍(95%置信区间为1. 319,4. 317)。结论:该院高血压特殊合并症患者的降压方案与指南推荐仍存在一定差距,依从性评价结果为医疗机构规范降压治疗提供较好的理论依据。更多还原

【Abstract】 Objective: To explore the compliance with guidelines for hypertension with special complications. Methods: Totally248 medical records diagnosed hypertension with special complications from January to December in 2017 were randomly selected. The compliance with guidelines was evaluated according to Guideline of Reasonable Drug Use in Hypertension (the 2ndedition). The influence of compliance on the rate of blood pressure target achievement was observed. Results: The rate of compliance with guidelines was 71. 77% and the rate of blood pressure target achievement was 68. 14%. The incompliance problems included start time of drug therapy,treatment options,drug choice,dosage and usage. Compared with that of the incompliance group,the target rate of the compliance group was 2. 34 times higher with 95% confidence interval of 1. 319-4. 317. Conclusion: There is still a certain gap between the antihypertensive program and the guideline recommendation for patients with special complications in the hospital. The results of compliance evaluation provide good theoretical basis for medical institutions to standardize the treatment of hypertension.更多还原
