夹板外固定和弹性髓内钉内固定方式治疗尺桡骨骨干骨折患儿的效果对比

【摘要】 目的:探索夹板外固定和弹性髓内钉内固定方式治疗尺桡骨骨干骨折患儿的效果。方法:选取2016年2月-2018年10月本院40例尺桡骨骨干骨折患儿,按照随机数字表法分为对照组和试验组,每组20例。对照组手法复位后给予夹板外固定,试验组手法复位后钛制弹性髓内针固定。随访10周后,比较两组功能恢复、X线评分、骨折愈合时间、骨性愈合时间、住院时间及术后不良反应发生率。结果:试验组的功能恢复情况明显优于对照组(P<0.05)。术后2、4、6、8周X线评分均明显优于对照组(P<0.05),而术后第10周两组X线评分比较,差异无统计学意义(P>0.05)。试验组骨折愈合时间、骨性愈合时间与住院时间均短于对照组(P<0.05),且试验组术后不良反应发生率明显低于对照组(P<0.05)。结论:采用弹性髓内针内固定可以有效固定儿童尺桡骨骨折,具有功能恢复快、治愈时间短,术后不良反应发生率低等优点,值得在临床大力推广。

【Abstract】 Objective:To explore the effect of splint external fixation and elastic intramedullary nailing for the treatment of ulnar and radial fractures in children.Method:40 children with ulnar and radial fractures admitted to our hospital from February 2016 to October 2018 were randomly divided into control group and experimental group according to the random number table method,20 casess in each group.The control group was treated with splint external fixation after manual reduction,and the experimental group was treated with titanium elastic intramedullary nailing fixation after manual reduction.After 10 weeks of follow-up,the functional recovery,X-ray score,fracture healing time,bone healing time,hospitalization time and incidence of postoperative adverse reactions were compared between the two groups.Result:The functional recovery of the experimental group was significantly better than that of the control group(P<0.05).The X-ray scores at 2,4,6 and 8 weeks after surgery in the experimental group were significantly better than those of the control group(P<0.05),while there was no significant difference in the X-ray scores at the 10 week after surgery between the two groups(P>0.05).The fracture healing time,bone healing time and hospitalization time in the experimental group were lower than those in the control group(P<0.05).The incidence of postoperative adverse reactions in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion:Elastic intramedullary nailing internal fixation can effectively fix the ulnar and radial fractures in children.It has the advantages of quick functional recovery,short healing time and low incidence of adverse reactions after operation.It is worthy of clinical application.
