【摘要】 目的:研究RockwoodⅢ型肩锁关节脱位患者采用微创下喙锁韧带带袢钢板(Endobutton重建喙锁韧带与锁骨钩钢板植入内固定的临床效果。方法:纳入本院2015年5月-2017年12月收治的86例RockwoodⅢ型肩锁关节脱位患者作为研究对象,按随机数字表法将患者分为对照组和微创组,各43例。对照组采用锁骨钩钢板植入内固定治疗,微创组采用微创下Endobutton带袢钢板重建喙锁韧带治疗。观察并比较两组患者的治疗效果、手术情况、术后恢复以及并发症情况。结果:微创组的治疗优良率为97.67%,高于对照组的83.72%,差异有统计学意义(P<0.05)。微创组手术时间略长于对照组,出血量少于对照组,住院时间短于对照组,差异均有统计学意义(P<0.05)。术前,两组患者疼痛、生活质量、关节活动范围以及肌力评分比较,差异均无统计学意义(P>0.05);术后,微创组患者疼痛、生活质量、关节活动范围以及肌力评分均高于对照组,差异均有统计学意义(P<0.05)。微创组并发症发生率仅为2.33%,明显低于对照组的13.95%,差异有统计学意义(P<0.05)。结论:Endobutton带袢钢板重建喙锁韧带对RockwoodⅢ型肩锁关节脱位患者治疗效果较好,虽然手术时间略微延长,但术中出血少,患者恢复快,术后恢复情况更佳,并发症明显减少,值得临床应用及推广。更多还原

【Abstract】 Objective:To study the effect of minimally invasive Endobutton plate on the reconstruction of coracoclavicular ligament or implanted clavicular hook plate in Rockwood Ⅲ acromioclavicular dislocation.Method:86 patients with Rockwood Ⅲ acromioclavicular dislocation treated from May 2015 to December 2017 in our hospital were selected.The patients were divided into control group and minimally invasive group by random number table method,43 cases in each group.The control group took the internal fixation with implanted clavicular hook plate,the minimally invasive group took the minimally invasive Endobutton plate on the reconstruction of coracoclavicular ligament.The therapeutic effect,operation conditions,postoperative recovery and complications were investigated.Result:The good rate in the minimally invasive group was 97.67%,significantly higher than83.72% in control group(P<0.05).The operation time of the minimally invasive group was slightly longer than that of the control group,the blood loss was less than that of the control group,and the hospitalization time was shorter than that of the control group,with statistically significant differences(P<0.05).Before surgery,there were no significant differences between the two groups in pain,quality of life,range of joint motion and muscle strength score(P>0.05);after surgery,pain,quality of life,range of joint motion and muscle strength score in the minimally invasive group were all higher than those in the control group,with statistically significant differences(P<0.05).The incidence of complications in the minimally invasive group was only 2.33%,significantly lower than 13.95% in the control group,the difference was statistically significant(P<0.05).Conclusion:Although the operation time is extended,the minimally invasive Endobutton plate on the reconstruction of coracoclavicular ligament can increase the therapeutic effect,reduce the bleeding volume and incidence rate of complications,and promote the fast recovery,it is worthy of clinical application and promotion.更多还原
