高灌注疗法在重型颅脑损伤术后脑梗死中的应用
Application of Hyperperfusion Therapy in Cerebral Infarction after Severe Craniocerebral Injury Surgery
【摘要】 目的:探讨高灌注疗法在重型颅脑损伤术后脑梗死中的应用效果。方法:选取本院2010年2月-2018年8月收治的重型颅脑损伤术后脑梗死患者60例为研究对象,按照随机数字表法将其分为对照组和观察组,各30例。对照组给予常规治疗,观察组在此基础上给予高灌注疗法治疗,治疗后CT复查梗死灶的变化情况,比较两组的临床疗效及神经功能缺损程度。结果:对照组治疗总有效率为23.33%明显低于观察组的70.00%,差异有统计学意义(x~2=13.125,P=0.003);且观察组的临床疗效明显优于对照组,差异有统计学意义(Z=-3.983,P=0.000)。治疗后,观察组的NIHSS评分情况明显优于对照组,差异有统计学意义(Z=3.476,P=0.001)。结论:高灌注疗法治疗重型颅脑损伤术后脑梗死患者可有效提高诊疗效果,减轻神经功能缺损程度,值得临床推广。
【Abstract】 Objective:To investigate the application effect of hyperperfusion therapy in cerebral infarction after severe craniocerebral injury surgery.Method:A total of 60 patients with cerebral infarction aftersevere craniocerebral injury surgery admitted io our hospital from February 2010 to August 2018 were selected as the research objects.According to the random number table method,they were divided into control group and observation group,30 cases in each group.The control group was given routine treatment,the observation group was given hyperperfusion therapy on this basis.After treatment,the changes of infarct focus were reexamined by CT,the clinical efficacy and the degree of neurological deficit between the two groups were compared. Result:The total effective rate of the control group was 23.33%,which was significantly lower than 70.00% of the observation group,the difference was statistically significant( x~2=13.125,P=0.003);the clinical efficacy of the observation group was significantly better than that of the control group,the difference was statistically significant(Z=-3.983,P=0.000).After treatment,the NIHSS score of the observation group was significantly better than that of the control group,the difference was statistically significant(Z=3.476,P=0.001). Conclusion:Hyperperfusion therapy for patients with cerebral infarction after severe craniocerebral injury surgery can effectively improve the diagnosis and treatment effect and reduce the degree of neurological deficit,which is worthy of clinical promotion.
