【摘要】 目的探讨单核细胞对急性胰腺炎(AP)诊断和分型鉴别价值。方法收集2014年5月-2017年5月天津市南开医院住院的急性胰腺炎患者521例,按疾病严重程度分为轻型(MAP)组、中重型(MSAP)组和重型(SAP)组,纳入同期非急性胰腺炎患者30例作为对照组,收集患者的一般资料和Mon%及Mon#,以经典指标"血糖"和"血钙"为对比进行统计与分析。结果①急性胰腺炎组与对照组之间Mon%差异无统计学差异(P>0.05),但Mon#、血钙(Ca)和血糖(Glu)差异有统计学意义(P<0.05);②SAP患者的Mon#低于MAP患者,Ca明显低于MAP及MSAP患者,Glu明显高于MAP及MSAP患者,Mon#(P<0.05)、Ca(P<0.01)、Glu(P<0.01);MSAP-1组患者的Mon#和Glu低于MSAP-2患者(P<0.05);③ROC曲线预测SAP曲线下面积为Ca(0.66/0.69)>Mon#(0.60/0.55)>Glu(0.59/0.66);ROC曲线预测MSAP亚类曲线下面积为Mon#(0.66)>Glu(0.62)>Ca(0.55)。结论 Mon#可能是可以辅助鉴别急性胰腺炎严重程度和亚类的有效指标。更多还原

【Abstract】 Objective To study the value of monocyte(Mon)in assessing acute pancreatitis(AP).Methods The analysis was conducted among 521 patients with AP in Tianjin Nankai Hospital from May,2014 to May,2017.According to the severity of disease,the cases were classified into mild acute pancreatitis group(MAP),moderatedly severe acute pancreatitis(MSAP)and severe acute panreatitis(SAP)groups,including 30 cases without AP as control group in the same period.The demographic datum and datum of monocyte percentage(Mon%)and monocyte absolute number(Mon#)of all cases were collected to evaluate the value of Monocyte with the analysis and comparison of blood calcium and glucose.Results Mon%,Ca and Glu were not statistically different between AP and the control group(P>0.05),while Mon#,Ca and Glu were statistically different between AP and the control group(P<0.05),Mon#in SAP group was lower than MAP group,Ca was significantly lower than MAP and MSAP groups,however Glu was significantly higher than MAP and MSAP groups,(Mon#,P<0.05.Ca,P<0.01.Glu,P<0.01).Mon# and Glu in MSAP-1 were significantly lower than MSAP-2 group(P<0.05).The areas under ROC curve for predicting SAP of Ca(0.66/0.69)were highest among the three.The areas under ROC curve for predicting MSAP sub-types were Mon#(0.66)>Glu(0.62)>Ca(0.55).Conclusion Mon# maybe a useful indicator to helpfully identify the severity and the sub-types of AP.更多还原
