血清脂联素在妊娠期高血压疾病中的变化及其与胰岛素抵抗的关系

【摘要】 目的探讨妊娠期高血压疾病(HDCP)患者血清脂联素(AFP)变化特点,分析其与胰岛素抵抗的关系。方法选择2014年1月至2018年1月我院诊治的160例HDCP患者为研究对象,依据HDCP分类标准将患者分为妊娠高血压组(63例)、轻度子痫前期组(50例)、重度子痫前期组(47例),并选取围产保健门诊产检的50例正常孕妇为对照组,采用酶联免疫吸附试验检测受试者血清AFP水平,稳态模型计算胰岛素抵抗指数(HOMA-IR),对比组间差异并分析AFP与HOMA-IR的关系。结果HDCP患者血清AFP水平为(6.59±1.44)μg/ml,对照组患者血清AFP水平为(11.57±3.15)μg/ml,两组比较,差异有统计学意义(P<0.05)。HDCP患者HOMA-IR为(2.61±0.92),对照组HOMA-IR为(1.02±0.37),两组比较,差异有统计学意义(P<0.05)。重度子痫前期组、轻度子痫前期组及妊娠高血压组AFP水平分别为(5.08±1.02)μg/ml、(6.15±1.27)μg/ml、(8.25±2.07)μg/ml,HOMA-IR水平分别为(3.02±1.03)、(2.51±0.89)、(1.34±0.45),HDCP不同病情程度患者血清AFP、HOMA-IR差异具有统计学意义(P<0.05),其中以重度子痫前期组AFP水平最低,HOMA-IR最高。胰岛素抵抗HDCP患者血清AFP水平为(5.14±1.95)μg/ml,低于非胰岛素抵抗HDCP患者的(9.36±2.88)μg/ml。Pearson相关分析血清AFP水平与HOMA-IR呈负相关(r=-0.54,P<0.05)。结论 HDCP患者血清AFP水平越低,胰岛素抵抗风险越大,AFP可以作为反映HDCP和胰岛素抵抗的有效指标。

【Abstract】 Objective To investigate the changes of serum adiponectin(AFP)in patients with gestational hypertension(HDCP),and to analyze the relationship between AFP and insulin resistance.Methods 160 cases of HDCP were selected from January 2014 to January 2018.According to the classification standard of HDCP,the patients were divided into pregnancy hypertension group(63 cases),mild preeclampsia group(50 cases)and severe preeclampsia group(47 cases).50 normal pregnant women in perinatal health clinic were selected as control group.Serum AFP levels were measured by enzyme-linked immunosorbent assay(ELISA),insulin resistance index(HOMA-IR)was calculated by steady-state model,and the relationship between AFP and HOMA-IR was analyzed.Results The serum AFP level of HDCP patients was(6.59±1.44)μg/ml,and that of control group was(11.57±3.15)μg/ml.There was significant difference between the two groups(P<0.05).The HOMA-IR of HDCP patients was(2.61±0.92)and that of control group was(1.02±0.37).The difference between the two groups was statistically significant(P<0.05).AFP levels in severe preeclampsia,mild preeclampsia and pregnancy-induced hypertension were(5.08±1.02)μg/ml、(6.15±1.27)μg/ml、(8.25±2.07)μg/ml,HOMA-IR levels in severe preeclampsia,mild preeclampsia and pregnancy-induced hypertension were(3.02±1.03)、(2.51±0.89)、(1.34±0.45),There were statistical significance(P<0.05).Among the severe preeclampsia group,the AFP level was the lowest and HOMA-IR was the highest.The level of serum AFP in insulin resistant HDCP patients was(5.14±1.95)μg/ml,which was lower than that in non insulin resistant HDCP patients(9.36±2.88)μg/ml.Pearson correlation analysis showed that serum AFP level was negatively correlated with HOMA-IR(r=-0.54,P<0.05).Conclusion The lower the level of serum AFP in HDCP patients,the greater the risk of insulin resistance,AFP can be used as an effective index to reflect HDCP and insulin resistance.
