θ短阵快速脉冲重复经颅磁刺激联合文拉法辛治疗首发抑郁症的临床效果及安全性
Clinical effects and safety of repetitive transcranial magnetic stimulation in TBS mode combined with Venlafaxine in treatment of first-episode depression
【摘要】 目的:观察θ短阵快速脉冲(TBS)模式重复经颅磁刺激(rTMS)联合文拉法辛治疗首发抑郁症的临床效果及安全性。方法:选取76例首发抑郁症患者作为研究对象,采用随机数字表法将其分为对照组和观察组各38例。对照组给予盐酸文拉法辛缓释胶囊治疗,观察组在对照组基础上联合TBS模式rTMS治疗。比较治疗前后两组心理状况的变化情况、不良反应发生情况及临床疗效。结果:治疗3、5周后,观察组汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评分均明显低于对照组,差异均有统计学意义(P<0.05);治疗前及治疗1、3、5周后,两组抗抑郁剂不良反应量表(SERS)评分比较,差异均无统计学意义(P>0.05);观察组治疗有效率为89.47%(34/38),明显高于对照组的71.05%(27/38),差异有统计学意义(P<0.05)。结论:TBS模式rTMS联合文拉法辛治疗首发抑郁症的临床效果明显优于单用文拉法辛,可快速有效改善患者的抑郁症状,且安全性好。
【Abstract】 Objective: To observe clinical effects and safety of repetitive transcranial magnetic stimulation(rTMS) in θ burst stimulation(TBS) mode combined with Venlafaxine in treatment of first-episode depression. Methods: 76 patients with first-episode depression were selectedas the study subjects, and they were divided into control group(n=38) and observation group(n=38) by using the random number table method.The control group was treated with Venlafaxine hydrochloride sustained-release capsules, while the observation group was treated with rTMS in theTBS mode on the basis of that of the control group. The changes of psychological status before and after the treatment, adverse reactions and clinicalefficacy were compared between the two groups. Results: 3 and 5 weeks after the treatment, the Hamilton depression scale(HAMD) and Hamiltonanxiety scale(HAMA) scores in the observation group were significantly lower than those in the control group, and the differences were statisticallysignificant(P<0.05). There were no statistical differences in the scores of rating scale for side effects of antidepressants(SERS) before and 1, 3 and 5 weeks after the treatment(P>0.05). The effective rate of the observation group was 89.47%(34/38), which was significantly higher than the controlgroup of 71.05%(27/38), and the difference was statistically significant(P<0.05). Conclusions: rTMS in the TBS mode combined with Venlafaxinein the treatment of first-episode depression is significantly better than single Venlafaxine. It can quickly and effectively improve the patients’ depressive symptoms and is safe.
