TGF-β轴在骨巨细胞瘤中的表达
Expression of TGF-β axis in giant cell tumor of bone
【摘要】 [目的]探讨TGF-β轴在骨巨细胞瘤中的表达。[方法] 2016年5月~2018年12月治疗的骨巨细胞瘤患者27例,所有患者均经病理组织检查确诊,患者均拟行手术治疗,术中取病灶组织及癌旁组织(距离病灶组织>3cm)。采用免疫组织化学法测定不同病灶组织中TGF-β轴表达水平;查阅病理资料,记录骨巨细胞瘤患者年龄、性别、肿瘤大小、影像学分期、病理学分级、复发、是否存在组织浸润、肿瘤部位,分析不同病理条件下TGF-β轴表达水平;采用多因素Logistic分析TGF-β轴影响因素。[结果]骨巨细胞瘤病灶组织中TGF-β轴蛋白阳性率高于癌旁组织(P<0.05)。免疫组化结果表明:TGF-β轴蛋白在骨巨细胞瘤中主要定位于单核间质细胞的细胞核中,而癌旁组织中表达水平较低或不表达。不同性别、年龄和肿瘤部位,TGF-β轴阳性表达的差异无统计学意义(P>0.05);而不同肿瘤直径、影像学分期、病理学分级、是否复发和是否存在组织浸润其骨巨细胞瘤病灶组织中TGF-β轴阳性表达差异有统计学意义(P<0.05)。多因素逻辑回归分析结果表明:骨巨细胞瘤病灶组织中TGF-β轴阳性表达与肿瘤直径、影像学分期、病理学分级、复发、是否存在组织浸润密切相关(P<0.05)。[结论] TGF-β轴在骨巨细胞瘤组织中呈高表达,且能反映患者的疾病严重程度,加强对其水平测定能评估预后,指导临床治疗。
【Abstract】 [Objective] To investigate the expression of TGF-β axis in giant cell tumor of bone. [Methods] A total of 27 patients with giant cell tumor of bone treated from May 2016 to December 2018 were enrolled into this study. All the patients were definitively diagnosed by pathological examination, and underwent surgical treatment. The lesion tissues and adjacent tissues more than 3 cm to the lesion were harvested during surgery. The expression of TGF-β axis in different lesions was determined by immunohistochemistry. In addition, the patients’ data were documented, including age, sex, tumor size, imaging stage, pathological grade, recurrence, presence of tissue infiltration, and tumor sites. The positive expression of TGF-β axis were analyzed by univariate and multivariate statistical methods. [Results] The positive expression of TGF-β-axis protein in the lesions of giant cell tumor proved higher than that in adjacent tissues(P<0.05). The immunohistochemistry showed that positive TGF-β-axis stains mainly localized in the nucleus of mononuclear interstitial cells in the lesion of giant cell tumor, whereas weak or no expression in adjacent tissues was found. In term of univariate analysis, the positive expression of TGF-β-axis proved of no statistical difference regarding to genders, age and tumor sites(P>0.05), whereas it was statistically different in the tumor diameter,imaging stage, pathological grade, recurrence or not, and presence of tissue infiltration or not(P<0.05). As results of the multivariate logistic analysis, the positive expression of TGF-β axis in the lesion of giant cell tumors proved significantly related to the tumor diameter, imaging stage, pathological grade, recurrence or not and presence of tissue infiltration or not. [Conclusion]The TGF-β axis highly expresses in giant cell tumor tissues, which might reflect the severity of the tumor, and might be used to evaluate the prognosis and guide clinical treatment.
